Abstract

The study includes the results of treatment of 167 patients with nephrolithiasis: first group included 80 patients with 1 session of extracorporeal shock wave lithotripsy (ESWL), the second group consisted of 87 patients who underwent 2-4 session ESWL. All patients underwent duplex scanning in renal blood flow, was determined by the concentration of serum creatinine and estimate the glomerular filtration rate. ELISA also investigated the dynamics of the content of lipocalin-2 (NGAL) and β2-microglobulin. All studies were performed at 5-7, 10-14, and 1-3 months after treatment. It is shown that in case of ESWL occurs renal parenchymal damage of varying degree, aggravated by increasing the number of sessions. By the early markers of acute kidney injury in patients with nephrolithiasis should include an increase urinary excretion of β2-MG and NGAL. Results of dynamic duplex scanning can reveal the underlying mechanisms of progression of renal dysfunction in patients after ESWL.

Abstract
In 171 patients with prostate cancer before radical prostatectomy assessed the frequency and nature of sexual disorders using the IIEF and hardness scale member. Erectile function was normal in only 73 (42,7%) respondents, 96 (57,3%) had reduced erection varying degrees, and in 74 (43,8%) patients had severe erectile dysfunction. A similar trend noted by studying the hardness of erection: 86 (54,9%) patients had an erection sufficient for sexual intercourse, but only 36 (21,1%) had erectile dysfunction at all. Orgasmic function was preserved in 111 (64,9%) of the surveyed patients, and in 89 (52%) of them was high. Level of erectile function depends on the patient's age and the presence of a concomitant cardiovascular disease, such as atherosclerosis and hypertension. Only 50,9% of patients show a high interest in restoring erectile function after surgery.

Abstract

Holmium laser enucleation of the prostate (HoLEP) is a minimally invasive procedure used for the treatment of benign prostatic hyperplasia. The method is as reliable performance with long-term monitoring, and high safety compared to conventional methods of surgical treatment of prostatic hyperplasia – suprapubic simple prostatectomy and transurethral resection of the prostate. Despite the complexity and duration of training for mastering HoLEP currently has a definite Frortage of references detailing the technical aspects of the method. The proposed article is detailed recommendations skilled mastering HoLEP. The material is based on a review of existing literature and my own experience and clinical material Latgale Urology Center in the development and conduct of HoLEP.

Abstract
During the last century various loop operations are used in the treatment of stress urinary incontinence. Methods for treating urinary incontinence are inextricably linked with the development of urology as a science. Dominance concept of sphincter deficiency as the main cause of urinary incontinence led to the use of various muscle loops, which was considered by many authors, could carry the functional load. The last decade was marked by significant developments and achievements in the treatment of urinary incontinence in women. Data received today entitle say that, in general, the use of synthetic loops installed without tension in the middle of the urethra, to get rid of urinary incontinence significant number of patients with this medical and social problem worldwide. We can say that with the advent of SIMS and their use in the treatment of urinary incontinence, a new stage. The concept of using SIMS for implantation which the surgeon requires one incision under the middle third of the urethra, is not innovative. So Smith presented his minisling made of pig skin in 1987. First available for use SIMS became minisling TVT Secur company Gynaecare (Ethicon) in 2005. To date, there are many SIMS: TVT Secur®, MiniArc®, Ophira®, Adjust®, Solyx®, Zipper Sling®, Prosurg®, Tissue Fixation System®, Needleless®, Altis®, Just-swing®, Desara®. Many manufacturers do not stop and continue to develop new minislingi with new locking mechanisms.

Abstract
In this review of the literature shows the epidemiology, classification, pathophysiology, diagnosis and treatments for erectile dysfunction in patients with type 1 diabetes. Erectile dysfunction in young patients with diabetes mellitus is a serious problem because of fairly widespread, and the impact on the psychological state of patients and quality of life in general. Emphasized that timely diagnosis with the correct definition of a form of erectile dysfunction can reasonably and adequately pick therapy for each individual patient.

Abstract
The article describes a clinical case of right renal artery anomalies in the form of «arterial forceps». Patient S., 26 years have seen a university clinic for two years. Complained of heaviness in the right lumbar region, not associated with urination and stoped after taking antispasmodics (drotaverin). In urinalysis revealed no pathological changes, serum creatinine, glomerular filtration rate in the normal range. Palpation in the right flank found kidney, easily movable hand pressure, painless. Ultrasound during supine and standing were identified nephroptosis, yavivsheesya indication for endoscopic surgery nephropexy. However, after the operation the patient's condition has not improved. Pain intensified and gained position hue. The study was revealed complex malformation of the vascular bed of the right kidney . It has been found that the right renal artery divides into two long branches before entering the gates kidney then forward it again bifurcated branch of the upper and lower branches and the same before entering the gates, forming a compression pelvis and causing its dilation.
