Повреждение яичковой артерии во время выполнения микрохирургической субингвинальной варикоцелэктомии (МСВ) в 20 – 40 % случаев приводит к атрофии яичка. Применение методик, облегчающих визуализацию яичковой артерии, позволило бы повысить эффективность и безопасность МСВ.</p></sec><sec><title>   Цель исследования</title><p>   Цель исследования. Оценить влияние применения интраоперационного допплеровского контроля (ИДК) на результаты микрохирургической субингвинальной варикоцелэктомии.</p></sec><sec><title>   Материалы и методы</title><p>   Материалы и методы. Ретроспективное когортное исследование. В исследование включён 101 пациент с клиническим левосторонним варикоцеле, которому в период с января 2022 года по ноябрь 2023 года в клинике «Мать и Дитя Ярославль» произведено хирургическое лечение. Стандартная микрохирургическая субингвинальная варикоцелеэктомия выполнена 52 пациентам (группа МСВ), 49 пациентам выполнена МСВ с интраоперационным допплеровским контролем (группа МСВ + ИДК). Для интраоперационного допплеровского контроля использовали «Минимакс-допплер-К» 20 МГц («СП Минимакс», Санкт-Петербург, Россия, 2023). Количество сохранённых артериальных и перевязанных венозных стволов подсчитывали интраоперационно. Наличие послеоперационных осложнений и изменения со стороны болевого синдрома оценивали через 3 месяца после операции.</p></sec><sec><title>   Результаты</title><p>   Результаты. Среднее время операции составило 79,5 ± 11,3 минут в группе МСВ и 75,4 ± 12,1 минут в группе МСВ + ИДК (p = 0,083). Несколько стволов яичковой артерии наблюдалось у 77,6 % пациентов в группе МСВ + ИДК и у 36,5 % в группе МСВ (p &lt; 0,001). Среднее количество перевязанных вен составило 16,7 ± 2,9 в группе МСВ+ИДК и 15,0 ± 3,8 в группе МСВ (p = 0,014). Количество сохраненных лимфатических сосудов не различалось между группами. Через 3 месяца после операции не отмечено развития таких осложнений, как рецидив варикоцеле, гидроцеле и атрофия яичка. Разрешение болевой симптоматики через 3 месяца после операции отмечено у 66,7 % и у 91,7 % пациентов с предоперационным болевым синдромом в группах МСВ и МСВ +ИДК соответственно (p = 0,047).</p></sec><sec><title>   Заключение</title><p>   Заключение. Применение интраоперационного допплеровского контроля при выполнении микрохирургической субингвинальной варикоцелэктомии позволяет сохранить большее количество стволов внутренней яичковой артерии, перевязать большее количество вен и получить лучшие показатели по разрешению болевого синдрома после операции.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>   Introduction</title><p>   Introduction. Injury to the testicular artery during microsurgical subinguinal varicocelectomy (MSV) results in testicular atrophy in 20 – 40% of cases. The use of techniques that improve visualization of the testicular artery could increase the efficacy and safety of MSV.</p></sec><sec><title>   Objective</title><p>   Objective. To evaluate the effect of intraoperative Doppler control (IDC) on the outcomes of microsurgical subinguinal varicocelectomy.</p></sec><sec><title>   Materials &amp; methods</title><p>   Materials &amp; methods. Retrospective cohort study. The study included 101 patients with clinical left-sided varicocele who underwent surgical treatment at the «Mother and Child Yaroslavl» clinic between January 2022 and November 2023. Fifty-two patients underwent standard microsurgical subinguinal varicocelectomy (MSV group), while 49 patients had MSV with intraoperative Doppler control (MSV + IDC group). The intraoperative Doppler control was performed using the Minimax-Doppler-K 20 MHz («SP Minimax», St. Petersburg, Russia). Intraoperatively, the number of preserved arterial and ligated venous stems was counted. Three months after the surgery, the presence of postoperative complications and changes in pain syndrome were assessed.</p></sec><sec><title>   Results</title><p>   Results. The average surgery time was 79.5 ± 11.3 minutes in the MSV group and 75.4 ± 12.1 minutes in the MSV + IDC group (p = 0.083). Multiple branches of the testicular artery were observed in 77.6 % of patients in the MSV + IDC group compared to 36.5 % in the MSV group (p &lt; 0.001). The mean number of ligated veins was 16.7 ± 2.9 in the MSV + IDC group versus 15.0 ± 3.8 in the MSV group (p = 0.014). No differences were found in the number of preserved lymphatic vessels between the groups. Three months after surgery, no complications such as varicocele recurrence, hydrocele, or testicular atrophy were reported.
