С 2006 по 2014 г. в нашей клинике наблюдались 8 больных с отрывами мочеточника после эндоскопических операций. Все больные были мужчинами, возраст их колебался от 50 до 71 года. У всех из них мочеточник был поврежден при уретероскопии с контактной литотрипсией и литоэктракцией. Его отрыв был диагностирован на границе верхней и средней трети у 2, ниже пиелоуретерального сегмента на 4 см у 4, полный отрыв от лоханки у 2 больных. У 7 из них травма мочеточника случилась в других стационарах. Им ранее была выполнена ревизия забрюшинного пространства и дренирование почки путем пиелонефростомии (6) и уретерокутанеостомии (1). В нашей клинике был один случай отрыва мочеточника, выполнена лапароскопическая нефрэктомия с аутотрансплантацией почки. Больному с уретерокутанеостомой слева установлен нефровезикальный шунт. У 6 пациентов с протяженными стриктурами мочеточника выполнена илеоуретеропластика. Реконструктивные операции и нефровезикальное шунтирование были успешными у всех больных. Осложнений в послеоперационном периоде не было. Среднее время кишечной пластики составило 160 мин, лапароскопической нефрэктомии с аутотрансплантацией –210 мин, нефровезикального шунтирования – 110 мин. Частой причиной отрыва мочеточника являются уретероскопия по поводу крупных длительно стоящих камней поясничного отдела мочеточника. В большинстве случаев после подобных травм развивается протяженная стриктура мочеточника, требующая проведения аутотрансплантации почки или илеоуретеропластики.</p></abstract><trans-abstract xml:lang="en"><p>Abstract. From 2010 to 2016 years we obsreved 8 patients with ureteral avulsion after endoscopic surgeries. All patients were men, their age ranged from 50 to 71 years. In all patients the ureter was damaged during ureteroscopy with lithotripsy and lithoextraction. Of the 8 patients, two  had proximal ureteral avulsion between upper and middle thirds, four had ureteral avulsion 4 cm away from the pyeloureteral junction , two had had complete pyeloureteral junction avulsion. In 7 cases trauma of ureter occured at other hospitals. These patients were underwent revision of retroperitoneal space and drain for kidney by nephrostomy (6) and ureterocutaneostomy (1). One case occurred at our department and this patient was underwent laparoscopic nephrectomy with autotransplantation. The patient with ureterocutaneostomy was treated by nephrovesical bypass. In 6 patients with extended ureteral strictures we performed ileoureteroplasty. Results of reconstructive surgeries and nephrovesical bypass were successful in all patients. No intra- and postoperative complications. Mean operative time during uleoureteroplasty was 180 min, laparoscopic nephrectomy with autotransplantation – 210 min, nephrovesical bypass – 110 min. Ureteral avulsion often associated with ureterosopy in patients with large impacted proximal ureteral stones.
