Травмы мочеточника составляют около 3% среди травм органов мочевой системы. До 38 – 80% травм мочеточника выявляется несвоевременно. Пропущенные уретеротравмы приводят к развитию осложнений.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Определение оптимальной тактики лечения ранних осложнений травм мочеточника.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В научно-исследовательском институте скорой помощи им. И.И. Джанелидзе с 2000 по 2022 год с ранними осложнениями, различными по механизму, виду и тяжести травм мочеточника, проходили лечение 46 пациентов. Тяжесть осложнений травм мочеточника определялась по классификации Clavien-Dindo. При лечении осложнений травм мочеточника применяли реконструктивные (в том числе этапные) операции.</p></sec><sec><title>Результаты</title><p>Результаты. По механизму повреждения мочеточника у 43 (93,5%) пациентов имелись ятрогенные травмы, у 2 (33,3%) — закрытые, у 1 (2,2%) — открытое ранение. По виду повреждения мочеточника различали разрыв, лигирование, раздавливание эндоклипсой и термическую травму. Вид уретеротравмы определял развитие ранних осложнений. Лигирование и раздавливание эндоклипсой мочеточника приводило к развитию почечной колики, гидронефрозу и обструктивному пиелонефриту. Разрыв и термическое повреждение мочеточника осложнялись мочевыми затеками и перитонитом. По классификации Clavien-Dindo, осложнения имели III b и IV степени тяжести. При лечении гнойно-воспалительных осложнений травм мочеточника выполнялись этапные операции. В остальных случаях гемодинамически стабильным пациентам с осложнениями уретеротравм проводили реконструктивные операции.</p></sec><sec><title>Заключение</title><p>Заключение. Выбор тактики лечения ранних осложнений травм мочеточника зависит от общего состояния пациента, наличия или отсутствия гнойно-воспалительного процесса, механизма, вида и локализации уретеротравмы.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Ureteral traumas account for approximately 3% of all urinary tract injuries. Up to 38 – 80 % of ureteral injuries are misdiagnosed. Consequently, missed ureteral injuries lead to the development of complications.</p></sec><sec><title>Objective</title><p>Objective. To determine the comprehensive management for early complications of ureteral injuries.</p></sec><sec><title>Materials &amp; methods</title><p>Materials &amp; methods. From 2000 to 2022, 46 patients with early complications of various mechanisms, types, and severity of ureteral injuries were treated at the Dzhanelidze Research Institute of Emergency Medicine. The severity of ureteral injury complications was determined using the Clavien-Dindo classification system. Reconstructive and staged operations were performed in the treatment of ureteral injury complications.</p></sec><sec><title>Results</title><p>Results. Fourty-three patients (93.5%) had iatrogenic injuries, 2 (33.3%) had blunt traumas, and 1 (2.2%) had an open injury. Ureteral injuries included rupture, ligation, endoclips compression, and thermal injury. The type of ureteral trauma determined the development of early complications. Ureteral ligation and endoclips compression led to the development of acute renal colic, hydronephrosis, and obstructive pyelonephritis. Ureteral rupture and thermal injury were complicated by urinary leaks and peritonitis. According to the Clavien-Dindo classification, the complications were IIIb and IV grade. Staged procedures were performed for the treatment of ureteral injuries complications in the presence of infection. In other cases, reconstructive operations were performed on hemodynamically stable patients with ureteral injury complications.</p></sec><sec><title>Conclusion</title><p>Conclusion.
