Возможность прогнозировать сроки купирования воспаления при обструктивном пиелонефрите на основании рутинных лабораторных показателей имеет большое значение для планирования следующего этапа лечения — устранения причины обструкции.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Оценить динамику и прогностическую ценность маркеров системной воспалительной реакции при лечении острого обструктивного пиелонефрита (ООП)</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведено проспективное исследование динамики воспалительных маркеров у пациентов с ООП. В исследование включены 113 пациентов с ООП, 43 из них была выполнена пункционная нефростомия и 70 — установка мочеточникового стента. Оценены уровни лейкоцитоза, СРБ, креатинина в день госпитализации и на 3-и и 7-е сутки после дренирования, длительность лихорадки, а также факторы риска длительной регрессии лейкоцитоза.</p></sec><sec><title>Результаты</title><p>Результаты. На 3-и сутки от начала лечения средний уровень лейкоцитов составил 10,5 (± 2,46) *109, а доля тех, у кого он нормализовался, превысила две трети — 68,1%. На 7-е сутки — 7,9 (± 3,12) *109 и 75,2% соответственно. Средний уровень СРБ на 3-и сутки — 14,2(± 63,95) мг/л, а доля имевших снижение до целевых значений — 32,7%. На 7-е сутки — 13,1 (± 49,7) мг/л и 56,6% соответственно. К 7-м суткам терапии лихорадка купировалась у 98% пациентов. Среди факторов, способных оказать влияние на длительное персистирование лейкоцитоза, статистически значимым (p &lt; 0,05) оказался сахарный диабет.</p></sec><sec><title>Заключение</title><p>Заключение. Исходя из величины стандартного отклонения маркеров воспаления в ходе лечения, лейкоцитоз может оцениваться как более специфичный маркер регрессии воспаления, чем СРБ. На 7-е сутки терапии три четверти пациентов демонстрируют лабораторные и клинические критерии купирования воспалительного процесса. Наличие у пациента сахарного диабета статистически значимо удлиняет регрессию воспалительной реакции при ООП.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Predicting the timing of inflammation control in acute obstructive pyelonephritis based on routine laboratory values is of great importance. This is essential for planning the next stage of management to eliminate the cause of obstruction.</p></sec><sec><title>Objective</title><p>Objective. To evaluate the dynamics and prognostic value of systemic inflammatory response markers in the management of acute obstructive pyelonephritis (AOP).</p></sec><sec><title>Materials &amp; methods</title><p>Materials &amp; methods. We performed a prospective study of the dynamics of inflammatory markers in patients with AOP. The study included 113 patients with AOP. Nephrostomy tube was installed in 43 and ureteral stent in 70. Following parameters were assessed: leukocytosis, CRP, creatinine on the days 1-3-7 after drainage, duration of fever. The risk factors for long-term regression of leukocytosis were also investigated.</p></sec><sec><title>Results</title><p>Results. On post-op day 3, the average leukocytosis was 10.5 (± 2.46) *109, and 68.1% of patients are returned to normal leukocyte level, and on post-op day 7 — 7.9 (± 3.12) *109 and 75.2%, respectively. On post-op day 3, the average CRP level was 14.2 (± 63.95) mg/l, and 32.7% of patients had decrease to target values, on post-op day 7 — 13.1 (± 49.7) mg/l and 56.6%, respectively. On post-op day 6, fever resolved in 98% of patients. Diabetes mellitus was the only statistically significant factor that can influence the long persistence of leukocytosis (p &lt; 0.05).</p></sec><sec><title>Conclusion</title><p>Conclusion. Leukocytosis showed higher specificity as a marker of inflammatory regression than CRP for AOP treatment. On post-op day 7, 75% of patients demonstrated laboratory and clinical signs for the inflammatory process relief.
