В мировой литературе практически отсутствуют работы, сравнивающие три техники резекции почки (открытой, лапароскопической и роботической) по двум критериям: «трифекта» и «пентафекта». Это обстоятельство определяет актуальность настоящего исследования.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Провести сравнительный анализ результатов применения трёх подходов к резекции почки по критериям «трифекта» и «пентафекта».</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В проспективное исследование включены 600 пациентов с раком почки, которым с 2018 по 2022 годы была выполнена резекция почки с помощью открытой (200 пациентов), лапароскопической (200 пациентов) и роботической (200 пациентов) техник. Результаты оценивали по «трифекте» (отрицательный хирургический край; время тепловой ишемии ≤ 25 минут или без ишемии; отсутствие послеоперационных осложнений по классификации Clavien-Dindo ≥ III в течение 3 месяцев после вмешательства) и «пентафекте» («трифекта», сохранение скорости клубочковой фильтрации &gt; 90% и отсутствие повышения стадии хронической болезни почек через 12 месяцев после операции).</p></sec><sec><title>Результаты</title><p>Результаты. «Трифекта» была достигнута после открытой, лапароскопической и роботической техник в 82%, 89% и 84% случаев соответственно. По данному критерию, между методиками достоверных различий не выявлено (p ˃ 0,05). «Пентафекта» была достигнута после открытой, лапароскопической и роботической техник в 53%, 64% и 66% наблюдений соответственно. По данному критерию, между открытой операцией и двумя остальными малоинвазивными подходами имело место наличие достоверных различий (p &lt; 0,05). При несложных для резекции опухолях (4 – 6 баллов по шкале R.E.N.A.L.) лучшие показатели «пентафекты» были характерны для лапароскопической и роботической техник. При умеренно сложных опухолях (7 – 9 баллов) для открытой операции были свойственны наихудшие показатели, но они значимо отличались только от результатов роботической техники (p &lt; 0,05). При самых сложных опухолях (10 – 12 баллов) худшие результаты показала лапароскопическая техника.</p></sec><sec><title>Заключение</title><p>Заключение. В целом все три техники резекции почки обеспечивают одинаковый результат по «трифекте», но по «пентафекте» лучшие результаты достижимы при малоинвазивных методиках (лапароскопической и роботической операциях). При стадиях сT1b и сT2а, а также при технически сложных для резекции опухолях (10 – 12 баллов по R.E.N.A.L.) в качестве техники выбора лечения следует отдавать предпочтение роботической РП.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. There is a dearth of literature comparing the three modalities of partial nephrectomy – open, laparoscopic, and robotic – based on two contemporary criteria, “trifecta” and “pentafecta”. This scarcity justiﬁes the signiﬁcance of this study.</p></sec><sec><title>Objective</title><p>Objective. To conduct a comparative evaluation of the outcomes of the three methods of partial nephrectomy, assessed against the criteria of “trifecta” and “pentafecta”.</p></sec><sec><title>Materials &amp; Methods</title><p>Materials &amp; Methods. The prospective study included 600 patients with renal cell cancer from 2018 to 2022. partial nephrectomy was performed using open (200 patients), laparoscopic (200 patients) and robotic (200 patients) techniques. Outcomes were assessed by “trifecta” (negative surgical margin; warm ischemia time ≤ 25 minutes or without ischemia; no ≥ Clavien-Dindo III grade postoperative complications within 3 months after surgery) and “pentafecta” (“trifecta”, ≥ 90% estimated glomerular ﬁltration rate preservation and no chronic kidney disease stage upgrading 12 months after surgery).</p></sec><sec><title>Results</title><p>Results. The “trifecta” outcome was achieved in 82%, 89%, and 84% of cases, respectively, using open, laparoscopic, and robotic approaches. No signiﬁcant diﬀerences in outcomes were found between these methods (p &gt; 0.05), according to this criterion. The “pentafecta” outcome was achieved in 53%, 64%, and 66% of cases using the same three approaches, respectively. Signiﬁcant diﬀerences in outcomes between the open approach and the minimally invasive techniques were observed (p &lt; 0.05) based on this criterion. For tumors that were considered easier to resect (R.E.N.A.L. 4 – 6 score), the highest “pentafecta” rates were observed with laparoscopic and robotic procedures. For tumors with moderate complexity (R.E.N.A.L. 7 – 9 score), open surgery resulted in the poorest outcomes, which were signiﬁcantly diﬀerent from those of robotic partial nephrectomy (p &lt; 0.05). The laparoscopic approach yielded the poorest results for the most complex tumors (R.E.N.A.L. 10 – 12 score).</p></sec><sec><title>Conclusions</title><p>Conclusions. In general, all three methods of partial nephrectomy produce the same outcome according to the “trifecta”, but according to the “pentafecta” better results may be achieved using minimally invasive techniques (laparoscopic and robotic procedures).
