Имеющиеся в настоящее время данные свидетельствуют о том, что лечение варикоцеле любой степени может улучшить мужскую фертильность. Но при этом диагностика варикоцеле 1 степени у мужчин с жалобами на бесплодие без использования УЗИ с допплерографией мошонки может привести к избыточной терапии.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Изучить сопоставимость и информативность визуально-пальпаторного метода с данными допплерографии мошонки в диагностике варикоцеле.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В период с ноября 2005 года по январь 2022 года в ГУ «РСНПМЦУ» прошёл обследование 2871 пациент с диагнозом «Варикоцеле и бесплодие», которым была выполнена микрохирургическая варикоцелэктомия чреспаховым доступом. Средний возраст пациентов составил 30,16 ± 0,09 (18 – 60) лет. Мужчины старшего возраста страдали вторичным бесплодием, обусловленным варикоцеле. Из 2871 пациента у 2592 (90,3%) диагноз был установлен на основании физикального метода обследования, а 279 (9,7%) пациентам — дополнительно выполнено УЗИ с допплерографией мошонки.</p></sec><sec><title>Результаты</title><p>Результаты. После внедрения в практику допплерографии мошонки в динамике доля пациентов с двусторонним варикоцеле увеличилось с 32,2% до 61,6% (р &lt; 0,01). Также достоверно увеличилось доля пациентов с правосторонним варикоцеле. Опытные врачи при физикальном обследовании пациентов с бесплодием у 4,3% не диагностировали варикоцеле 1 степени.</p></sec><sec><title>Заключение</title><p>Заключение. С внедрением в диагностику мужского бесплодия УЗИ с допплерографией мошонки улучшилась выявляемость варикоцеле 1 степени, а также двухстороннее варикоцеле, что в свою очередь способствовало более раннему устранению мужского фактора бесплодия у данного контингента пациентов. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Current evidence suggests that management of any grade of varicocele can improve male fertility. However, diagnosis of grade 1 varicocele in infertile men without the use of scrotal Doppler ultrasound may cause overtreatment.</p></sec><sec><title>Objective</title><p>Objective. To study the comparability and accuracy of the visual palpatory examination method with the scrotal Doppler ultrasound data for the diagnosis of varicocele.</p></sec><sec><title>Material &amp; methods</title><p>Material &amp; methods. Between November 2005 to January 2022, 2871 patients diagnosed with varicocele and infertility who underwent microsurgical varicocelectomy by inguinal access were examined at RSSPMCU. The average age of the patients was 30.16 ± 0.09 (18 – 60) years. Of the 2871 patients, 2592 (90.3%) were diagnosed by physical examination, and 279 (9.7%) patients underwent additional scrotal Dopper ultrasound.</p></sec><sec><title>Results</title><p>Results. After Doppler scrotal imaging was introduced into practice, the proportion of patients with bilateral varicocele increased from 32.2% to 61.6% (p &lt; 0.01). The proportion of patients with right-sided varicocele also increased signiﬁcantly. Experienced physicians during physical examination of infertility patients did not diagnose grade 1 varicocele in 4.3%.</p></sec><sec><title>Conclusions</title><p>Conclusions. The implementation of scrotal Doppler ultrasound for the diagnosis of male infertility improved the detection of grade 1 varicocele and bilateral varicocele.
