Рак предстательной железы занимает ведущие позиции в структуре онкологической заболеваемости среди мужчин во всем мире. “Золотым стандартом” лечения локализованных форм рака простаты является радикальная простатэктомия, при этом в последние годы всё чаще оперативное пособие выполняется робот-ассистированно. Одним из негативных последствий операции для пациента является возникновение недержания мочи, что встречается достаточно часто и существенно снижает качество жизни. По этой причине крайне важно выделить факторы, которые влияют на возникновение недержания мочи.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Выявить факторы риска возникновения недержания мочи у больных, перенёсших робот-ассистированную простатэктомию.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В клинике урологии Российского Университета Медицины проведено проспективное когортное исследование по изучению факторов риска недержания мочи у пациентов, перенёсших робот-ассистированную радикальную простатэктомию. Контрольными точками наблюдения были установлены 1-е, 3-и, 7-е сутки после удаления уретрального катетера и на 1-й, 3-й, 6-й, 12-й месяцы после операции. Проанализированы анамнестические данные и результаты инструментальных методов исследования и проведён статистический анализ, включающий логистический регрессионный анализ с построением уравнения регрессии.</p></sec><sec><title>Результаты</title><p>Результаты. В раннем послеоперационном периоде недержание мочи возникло у 87 (70,3%) пациентов, но к 12-му месяцу наблюдения его частота снизилась до 15 (11%) случаев. Возраст, индекс массы тела, длина мембранозной уретры, размер m. levator ani, перенесённые операции на предстательной железе в анамнезе и симптомы гиперактивного мочевого пузыря являлись предикторами недержания мочи. На основе результатов созданы номограммы для расчёта вероятности недержания мочи на 7-е сутки и через 1, 3, 6, 12 месяцев после удаления уретрального катетера.</p></sec><sec><title>Заключение</title><p>Заключение. Отрицательное влияние инконтиненции на качество жизни пациентов перенёсших РПЭ очевидно. Среди выявленных нами предикторов выделена модифицируемая группа факторов риска недержания мочи. Это позволяет снизить риск и степень тяжести недержания мочи после операции, что благотворно скажется на качестве жизни после операции.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Prostate cancer (PCa) occupies a leading position in the structure of cancer incidence among men worldwide. The “gold standard” for the treatment of localised PCa is radical prostatectomy (RPE), with an increasing frequency of robot-assisted surgery in recent years. One of the negative consequences of surgery for the patient is the occurrence of urinary incontinence (UI), which is quite common and significantly reduces the quality of life. For this reason, it is extremely important to identify the factors that influence the occurrence of UI.</p></sec><sec><title>Objectve</title><p>Objectve. To identify risk factors for urinary incontinence in patients undergoing robot-assisted surgery based on the patients' preoperative data.</p></sec><sec><title>Materials &amp; methods</title><p>Materials &amp; methods. A prospective cohort study was conducted at the Urology Clinic of the Russian University of Medicine to study the risk factors for UI in patients undergoing robot-assisted RPE. The follow-up control points were set on days 1, 3, and 7 after urethral catheter removal and at 1, 3, 6, and 12 months after surgery. Anamnestic data and results of instrumental examination were analysed, based on which statistical analysis including logistic regression analysis with construction of regression equation was performed.</p></sec><sec><title>Results</title><p>Results. In the early postoperative period, UI occurred in 87 (70.3%) patients, but by the follow-up month 12, its frequency had decreased to 15 (11%) cases. Age, body mass index, length of the membranous urethra, size of the m. levator ani, history of prostate surgery and symptoms of an overactive bladder were predictors of UI. Based on the results, nomograms were created to calculate the probability of UI on day 7 and 1, 3, 6, 12 months after urethral catheter removal.</p></sec><sec><title>Conclusion</title><p>Conclusion. The negative impact of UI on the quality of life of patients who have undergone RPE is obvious. Among the predictors identified by us, a modifiable group of risk factors for urinary incontinence was identified.
