В настоящее время Erector Spinae Plane (ESP) block — блокада мышцы, выпрямляющей спину, — получила широкое распространение как компонент мультимодальной анестезии, так как создаёт обширный сенсорный блок, распространяющийся на 5 – 7 сегментов.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Оценить эффективность блокады ESP в качестве компонента мультимодальной анестезии вовремя люмботомических операций на почках.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Исследования проведены у 116 пациентов в возрасте от 18 – 85 лет I – III класса по ASA (American Society of Anesthesiologists / Американское общество анестезиологов). Всем пациентам проводили ингаляционную анестезию Изофлюраном. В качестве упреждающей аналгезии вводили НПВС (Диклофенак 75 мг в/м). В исследуемой группе (60 пациентов) после интубации трахеи и позиционирования под УЗ-контролем выполняли ESP block на уровне Th-9 0,5% р-ром Бупивакаина (25 – 30 мл). В контрольной группе (56 пациентов) проводили только ингаляционную анестезию. В послеоперационном периоде оценивали выраженность боли, частоту потребления опиатов (Промедол), послеоперационную тошноту и рвоту. Для оценки боли использовали цифровую рейтинговую шкалу (ЦРШ), регистрацию которой проводили каждые 6 часов в течение суток.</p></sec><sec><title>Результаты</title><p>Результаты. Пациенты в группе с ESP блоком показали низкую оценку послеоперационной боли по ЦРШ в сравнении с контрольной группой (p &lt; 0,001), потребление опиатов также оказалось ниже, особенно в первые 6 часов после операции (p &lt; 0,001).</p></sec><sec><title>Заключение</title><p>Заключение. Результаты исследований подтвердили эффективность ESP блокады, что привело к снижению потребления пациентами опиатов и НПВС. Основным преимуществом этого метода является простота выполнения и его безопасность.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Currently, the Erector Spinae Plane (ESP) block is widely used both as an independent method of anesthesia and as a component of multimodal anesthesia. It creates an extensive sensor block, spreads over 5 – 7 spinal segments.</p></sec><sec><title>Objective</title><p>Objective. To evaluate the efficacy of ESP block as a component of multimodal anesthesia during open kidney surgery.</p></sec><sec><title>Material &amp; methods</title><p>Material &amp; methods. The studies were conducted on 116 patients of ASA class I – III at the age of 18 – 85 years All patients were performed inhalation anesthesia with Isoflurane. NSAID (Diclofenac 75 mg IM) was administered as pre-emptive analgesia. In the Study group (n = 60), after tracheal intubation and positioning, an ESP block was performed under ultrasound control at the Th-9 level with a 0.5% Sol. Bupivacaine (25 – 30 ml). In the Control group (n = 56), only inhalation anesthesia was performed. Pain severity, opioid consumption frequency (Sol. Promedoli), postoperative nausea and vomiting were assessed in the postoperative period. A numeric rating scale (NRS) was used to assess pain, which was recorded every 6 hours during the day.</p></sec><sec><title>Results</title><p>Results. Patients in the study group showed a low assessment of postoperative pain on the numeric rating scale compared to the Control group (p &lt; 0.001). The consumption of opiates (Sol. Promedoli) was also lower, especially in the first 6 hours after surgery (p &lt; 0.001).</p></sec><sec><title>Conclusions</title><p>Conclusions. The results of our studies confirmed the efficiency of ESP block by reducing the consumption of opiates and NSAIDs.
