В мировой литературе практически отсутствуют работы по изучению вопроса лечения пациентов с сочетанием стриктуры передней уретры и доброкачественной гиперплазии предстательной железы (ДГПЖ).</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Проанализировать результаты начального опыта симультанного лечения стриктуры передней уретры и ДГПЖ.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование включены четыре пациента с сочетанием стриктуры передней уретры и ДГПЖ, у которых проводили симультанное оперативное лечение в объёме промежностной / пенильной уретростомии и тулиевой лазерной энуклеации гиперплазии предстательной железы. Критерием успешности лечения считали отсутствие обструктивных симптомов нижних мочевых путей, потребовавших применения эндоскопического или открытого хирургического вмешательства, вне зависимости от того, что уретростомия рассматривается как первый этап или окончательный вариант деривации мочи.</p></sec><sec><title>Результаты</title><p>Результаты. Возраст пациентов варьировался от 54 до 69 лет (среднее значение — 63,0 ± 6,5 лет). У одного (25%) пациента стриктура была локализована в пенильной уретре, у трёх (75%) — в бульбозной уретре. Протяжённость стриктур колебалась от 1,0 до 3,0 см (среднее значение — 2,1 ± 0,9 см). Объём предстательной железы составлял от 45 до 77 см3 (среднее значение — 63,8 ± 13,5 см3). В одном наблюдении имело место наличие цистостомического дренажа. Объём остаточной мочи среди трёх пациентов с сохранённым самостоятельным мочеиспусканием составлял от 90 до 130 мл (среднее значение — 113,3 ± 20,8 мл). Сроки послеоперационного наблюдения варьировались от 3 до 18 месяцев (среднее значение — 11,3 ± 7,4 месяцев). При данных сроках наблюдения лечение было успешным у всех 4 (100%) пациентов. Каких-либо интра- и послеоперационных осложнений не отмечено.</p></sec><sec><title>Заключение</title><p>Заключение. Предложенный нами способ может быть успешно применён у пациентов с сочетанием стриктуры передней уретры и ДГПЖ. Однако окончательные рекомендации по тактике ведения таких пациентов могут быть разработаны после дальнейших научных исследований.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Research on the treatment of patients with a combination of anterior urethral stricture and benign prostatic hyperplasia (BPH) is basically absent in the worldwide literature.</p></sec><sec><title>Objective</title><p>Objective. To analyse the primary experience results with simultaneous treatment of anterior urethral stricture and BPH.</p></sec><sec><title>Materials &amp; methods</title><p>Materials &amp; methods. The study included four patients with anterior urethral stricture and BPH, who underwent simultaneous surgical treatment including perineal / penile urethrostomy and thulium laser enucleation of the prostate. The success criterion for treatment was considered to be the absence of obstructive lower urinary tract symptoms requiring the use of endoscopic or open surgery, regardless of whether urethrostomy was considered as the first step or the final option for urinary diversion.</p></sec><sec><title>Results</title><p>Results. The age of the patients ranged from 54 to 69 years (mean 63.0 ± 6.5 years). In one (25%) patient the stricture was localised in the penile urethra and in three (75%) patients it was localised in the bulbous urethra. The stricture length ranged from 1.0 to 3.0 cm (mean 2.1 ± 0.9 cm). The prostate volume varied from 45 to 77 cc (mean 63.8 ± 13.5 cc). One patient had cystostomy drainage. The post-void residual volume among 3 patients with preserved urination ranged from 90 to 130 ml (mean 113.3 ± 20.8 ml). Postoperative follow-up periods ranged from 3 to 18 months (mean 11.3 ± 7.4 months). At these follow-up periods, treatment was successful in all four (100%) patients. No intra- or postoperative complications were observed.</p></sec><sec><title>Conclusions</title><p>Conclusions. The proposed method can be successfully applied in patients with a combination of anterior urethral stricture and BPH.
