Радикальная простатэктомия (РПЭ) — «золотой стандарт» лечения пациентов с раком предстательной железы (РПЖ). Одним из активно изучаемых предикторов биохимического рецидива является наличие положительного хирургического края (ПХК).</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Разработать и первично оценить методики таргетного (прицельного) срочного интраоперационного гистологического исследования свежезамороженных срезов при выполнении радикальной простатэктомии.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В Университетской клинике урологии РНИМУ им. Н.И. Пирогова на базе ГКБ № 1 им. Н.И. Пирогова проведено проспективное клиническое исследование по исследованию свежезамороженных срезов во время выполнения РПЭ. В период с февраля 2021 года по апрель 2022 года в исследование включено 112 пациентов с диагнозом РПЖ, которым одним хирургом была выполнена лапароскопическая РПЭ (экстра- и трансперитонеальным доступами). В группу А (62 человека) вошли пациенты, которым на основании предоперационных данных (МРТ, FUSION биопсия) выполнено интраоперационное исследование свежезамороженных срезов. Группа Б (50 человек) — контрольная группа (ретроспективная), в которую вошли пациенты со стандартным выполнением РПЭ без срочного гистологического исследования.</p></sec><sec><title>Результаты</title><p>Результаты. Онкологический процесс в макропрепаратах, полученных при выполнении cito гистологии, был обнаружен у 42 (67,7%) пациентов, первично ПХК — у 12 (19,4%). В группе Б (контрольная группа, без выполнения cito гистологии) ПХК выявлен в 7 (14%) случаях. Миграция статуса ХК из первично негативно в окончательно позитивный установлена у 2 (3,3%) пациентов и, напротив, из первично положительного в отрицательный ХК — у 2 (3,3%). Истинно негативный ХК определён у 48 из 50 пациентов, а истинно положительный ХК — у 10 из 12. Таким образом, специфичность и чувствительность методики составили 96,1% и 83,3% соответственно. Среднее время выполнения срочного гистологического исследования составило 24 минуты.</p></sec><sec><title>Заключение</title><p>Заключение. Полученные результаты указывают на эффективность и целесообразность использования таргетного (прицельного) cito-гистологического исследования во время выполнения РПЭ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Radical prostatectomy (RP) is the "gold standard" treatment for patients with prostate cancer (PCa). Obviously, one of the predictors actively studied of biochemical recurrence is the presence of a positive surgical margin.</p></sec><sec><title>Objective</title><p>Objective. To develop and initially evaluate the techniques of targeted urgent intraoperative histological study of fresh frozen sections during radical prostatectomy.</p></sec><sec><title>Materials &amp; methods</title><p>Materials &amp; methods. This study was carried out within the framework of the academic research at the University Clinic of Urology, Pirogov Medical University. We conducted a prospective clinical study of fresh frozen tissue samples during laparoscopic RP (LPR). The subject of the investigation are 112 patients with PCa underwent LRP (extra- and transperitoneal approaches) by one surgeon in 2021 – 2022. Group A involves 62 patients underwent intraoperative examination of fresh frozen sections based on preoperative data (MRI, FUSION biopsy). Group B (control retrospective group) consists of 50 patients with standard surgery without fresh frozen sections.</p></sec><sec><title>Results</title><p>Results. The accumulation of new data allows us to say that the presence of an oncological process was found in the <ext-link xlink:href="https://www.multitran.com/m.exe?l1=1&amp;l2=2&amp;s=gross+specimen&amp;split=1" ext-link-type="uri">gross specimen</ext-link> obtained by performing cito histology in 42 (67.7%) patients. A primary positive margin was found in 12 (19.4%). Compared to group B, a positive surgical margin was found in 7 (14%). Migration of surgical margin status from initially negative to finally positive margin found in 2 (3.3%). Migration of status from primary positive to negative surgical margin was found in 2 (3.3%). The specificity of the method (true negative margin) was determined in 48 of 50 patients (96.1%), and the sensitivity (true positive margin) was determined in 10 of 12 patients (83.3%). The average time to complete the technique of rapid histological study was 27 minutes.</p></sec><sec><title>Conclusion</title><p>Conclusion.
