Согласно клиническим рекомендациям, максимальный диаметр камня является одним из основных критериев в выборе метода нефролитотрипсии. При планировании операции хирург ориентируется не только на максимальный диаметр, но и на другие параметры: анатомию ЧЛС, плотность камня, их количество, наличие каликопиелоэктазии, операции в анамнезе и т.д. Максимальный диаметр камня не является исчерпывающей характеристикой, позволяющей подобрать оптимальную тактику лечения.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Оценить влияние объёма камня (по сравнению с его максимальным диаметром) на продолжительность минимально инвазивной эндоскопической нефролитотрипсии.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Исследование носило ретроспективный характер. В исследование включено 55 пациентов (22 женщины и 33 мужчины), средний возраст — 47,0 ± 1,9 лет. Всем пациентам выполнена тулиевая нефролитотрипсия по поводу камней до 20 мм (средний максимальный диаметр — 13,3 ± 0,6 мм, средняя плотность — 1041,0 ± 48,0 HU) путём ретроградной интраренальной хирургии (РИРХ, n = 30), миниперкутанной и микроперкутанной нефролитотрипсий (миниПНЛ, n = 16 и микроПНЛ, n = 9, соответственно). Исключались из исследования пациенты с аномалиями мочевой системы, мочевой инфекцией в стадии обострения и пациенты, у которых не было достигнуто состояние, свободное от камней. Помимо оценки стандартных показателей, таких как максимальный диаметр, плотность конкремента и его локализация, у всех пациентов был рассчитан объём конкремента по формуле неравностороннего эллипсоида (медианный объём камня — 287 [144; 538] мм³). Коэффициент ранговой корреляции Spearman (r) с оценкой уровня значимости был рассчитан для максимального диаметра и объёма конкремента для общей выборки пациентов и для каждого метода оперативного вмешательства по отдельности.</p></sec><sec><title>Результаты</title><p>Результаты. При анализе общей выборки пациентов достоверно выявлена слабая корреляция (r = 0,39) между максимальным диаметром камня и временем операции. А между объёмом камня и временем операции выявлена умеренная корреляция (r = 0,53). Аналогичная связь с линейным распределением прослеживалась и при анализе в группах различных методов малоинвазивной нефролитотрипсии.</p></sec><sec><title>Заключение</title><p>Заключение. При выборе малоинвазивной лазерной нефролитотрипсии целесообразно ориентироваться не только на максимальный диаметр, плотность камня и его локализацию, но и на объём конкремента, который имеет большую связь со временем операции.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. According to the guidelines, the stone maximum diameter is one of the main criteria for choosing the method of nephrolithotripsy. When planning an operation, the surgeon focuses not only on the diameter, but also on the renal pelvis anatomy, stone density and number of it, the presence of hydronephrosis, a history of surgery, etc. The maximum diameter is not an exhaustive characteristic that allows you to choose the optimal treatment.</p></sec><sec><title>Objective</title><p>Objective. To evaluate the effect of stone volume (compared to its maximum diameter) on the duration of minimally invasive endoscopic nephrolithotripsy.</p></sec><sec><title>Materials &amp; methods</title><p>Materials &amp; methods. The study was retrospective. The study included 55 patients (22 women, 33 men), the average age was 47.0 ± 1.9 years. All patients underwent minimally invasive thulium fiber laser nephrolithotripsy for stones up to 20 mm (mean maximum diameter — 13.3 ± 0.6 mm, mean density — 1041.0 ± 48.0 HU). Minimally invasive endoscopic interventions such as retrograde intrarenal surgery (RIRS, n = 30), minipercutaneous and micropercutaneous nephrolithotripsy (miniPNL, n = 16 and microPNL, n = 9, respectively) were performed. Patients with urinary system abnormalities, acute urinary tract infections and patients without stone-free status were excluded from the study. In addition to evaluating standard indicators, the stone volume was calculated in all patients using the formula of a scalene ellipsoid (median volume — 287 [144; 538] mm3). Spearman's rank correlation coefficient (r) with an assessment of the significance level was calculated for the stone maximum diameter and volume for the total sample of patients and for each surgical intervention method separately.</p></sec><sec><title>Results</title><p>Results. The analysis of the total sample of patients reliably revealed a weak correlation (r = 0.39) between the stone maximum diameter and surgery time. And a moderate correlation was found between the stone volume and surgery time (r = 0.53). A similar relationship with the linear distribution was also observed in the analysis in all groups.</p></sec><sec><title>Conclusion</title><p>Conclusion.
