Преждевременная эякуляция (ПЭ) является второй по распространённости мужской сексуальной дисфункцией с выраженным негативным влиянием на качество жизни. Предлагается значительное количество консервативных и оперативных методов лечения ПЭ, однако их эффективность, а в некоторых случаях и безопасность, до сих пор служат предметом дискуссий.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Изучить эффективность пульсовой радиочастотной импульсной нейромодуляции (РЧН) с различными режимами воздействия на дорсальный нерв полового члена при лечении пациентов с преждевременным семяизвержением.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В проспективном многоцентровом исследовании участвовали две группы пациентов с ПЭ: 1 группа — 11 пациентов в возрасте 22 – 34 года; 2 группа — 12 пациентов а возрасте 23 – 34 года. В группе 1 применена пульсовая РЧН при температуре 42 °С, частоте стимуляции 2 Гц, 45 V и продолжительности 180 секунд; в группе 2 — пульсовая РЧН при температуре 50 °С и аналогичными с группой 1 частотой и продолжительностью стимуляции. Все пациенты до и после лечения измеряли время интравагинальной задержки семяизвержения (ВИЗС) и заполняли профиль преждевременной эякуляции (ППЭ). Оценку результатов осуществляли через 30 дней.</p></sec><sec><title>Результаты</title><p>Результаты. В обеих группах отмечено статистически значимое увеличение ВИЗС и ППЭ. Так, в группе 1 ВИЗС увеличилось с 58 до 101,9 секунд, ППЭ — с 3,8 до 12,6 баллов. В группе 2 ВИЗС увеличилось с 66,5 до 210,9 секунд, ППЭ — с 3,2 до 12,6 баллов. При этом, в группе 2 ВИЗС увеличилось на 107% больше, чем в группе 1. Значимых побочных эффектов не отмечено в обеих группах.</p></sec><sec><title>Заключение</title><p>Заключение. Радиочастотная пульсовая нейромодуляция представляется весьма перспективным способом лечения преждевременного семяизвержения, поскольку является малоинвазивной, безопасной и обратимой процедурой. Более эффективной выглядит методика воздействия на дорсальный нерв при температуре электрода 50 °С по сравнению с 42 °С, однако необходимы дополнительные исследования.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction.  Premature ejaculation (PE) is the second most common male sexual dysfunction with a significant negative impact on quality of life. A significant number of conservative and surgical methods for the treatment of PE are proposed, but their effectiveness and, in some cases, safety, remain the subject of discussion.</p></sec><sec><title>Objective</title><p>Objective. To study the effectiveness of pulsed radiofrequency impulse neuromodulation (RPN) with different modes on the penile dorsal nerve in treatment of patients with premature ejaculation.</p></sec><sec><title>Materials &amp; methods</title><p>Materials &amp; methods. Two groups of PE-patients participated in a prospective multicenter study: Group 1 — 11 patients aged 22 – 34 years; Group 2 — 12 patients aged 23 – 34 years. Group 1 received pulsed RPN at a temperature of 42°C, a stimulation frequency of 2 Hz, 45 V, and a duration of 180 seconds; in Group 2 — pulsed RPN at a temperature of 50 °C and the same frequency and duration of stimulation as Group 1. All patients before and after treatment measured intravaginal ejaculation latency time (IELT) and completed a premature ejaculation profile (PEP). The results were evaluated after 30 days.</p></sec><sec><title>Results</title><p>Results. In both groups, there was a statistically significant increase in IEDT and PEP. Thus, in Group 1, IELT increased from 58 to 101.9 seconds, PEP from 3.8 to 12.6 points. In Group 2, IELT increased from 66.5 to 210.9 seconds, PEP from 3.2 to 12.6 points. At the same time, in Group 2, IELT increased by 107% more than in Group 1. No significant side effects were observed in both groups.</p></sec><sec><title>Conclusion</title><p>Conclusion. RPN appears to be a very promising treatment for premature ejaculation, as it is a minimally invasive, safe, and reversible procedure.
