На протяжении более 30 лет трансректальная биопсия под ультразвуковым контролем (ТРУЗИ) являлась стандартным методом диагностики рака предстательной железы (РПЖ). При этом стандартная биопсия не лишена всем известных недостатков. С целью их устранения и улучшения диагностики клинически значимого РПЖ предложены магнитнорезонансно(МР)-прицельные методы биопсии. Наибольшее распространение в клинической практике получили когнитивная и мпМР/УЗ-фьюжн биопсия. Однако при их сравнении получены противоречивые данные по выявляемости клинически значимого РПЖ.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Сравнить выявляемость клинически значимого рака предстательной железы при проведении мпМР/УЗ-фьюжн и когнитивной биопсии.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Критерии включения: ПСА &gt; 2,0 нг/мл и / или положительное ПРИ, и / или подозрительный участок на ТРУЗИ, наличие очага PI-RADSv2.1 ≥ 3. Оцениваемые результаты: выявляемость клинически значимого РПЖ (ISUP ≥ 2), общая выявляемость РПЖ, выявляемость клинически незначимого РПЖ (ISUP 1), гистологическая эффективность (доля положительных биоптатов, длина поражения раком биоптата).</p></sec><sec><title>Результаты</title><p>Результаты. Проведён ретроспективный анализ данных: 102 пациентам выполнена когнитивная биопсия и 176 пациентам — мпМР/УЗ-фьюжн биопсия. Медиана возраста — 63 года, объёма простаты — 46 см3. Медиана уровня ПСА — 6,4 нг/мл в группе мпМР/УЗ-фьюжн биопсии и 6,7 нг/мл в группе когнитивной биопсии. При сравнении мпМР/УЗ-фьюжн и когнитивной биопсии выявляемость клинически значимого рака (30,3% и 25,0%; p = 0,329) и общая выявляемость рака (50,5% и 42,1%; p = 0,176) сопоставимы. При помощи когнитивной биопсии клинически незначимый рак выявляли реже по сравнению с мпМР/УЗ-фьюжн биопсией (11,8% и 25,5%; p = 0,007). Доля положительных биоптатов между когнитивной и мпМР/УЗ-фьюжн биопсией (30,5% и 29,5%; p = 0,754) и максимальная длина биоптата, поражённого раком (6,6 мм и 7,6 мм; p = 0,320) также сравнимы. При этом доля положительных биоптатов с клинически значимым РПЖ больше в группе когнитивной биопсии (18,6% и 13,1%; p = 0,029).</p></sec><sec><title>Заключение</title><p>Заключение. Как когнитивная, так и мпМР/УЗ-фьюжн биопсия являются одинаково точными методами диагностики клинически значимого РПЖ. В связи с этим когнитивная биопсия может являться альтернативой мпМР/УЗ-фьюжн в лечебных учреждениях, где технология мпМР/УЗ-фьюжн биопсии в настоящее время недоступна.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Transrectal biopsy under US-control has been standard diagnostic method for prostate cancer (PCa) detection for over 30 years. However, TRUS-guided biopsy is not without well-known drawbacks. MR-targeted biopsy methods were proposed to eliminate the drawbacks and improve detection rate of clinically significant Pca. Cognitive and mpMR/US-fusion biopsies have become the most widely used MR-targeted biopsies. However, there are contradictory data on detection of clinically significant Pca when comparing mpMR/US-fusion and cognitive biopsies.</p></sec><sec><title>Objective</title><p>Objective. To compare the detection rate of clinically significant prostate cancer performing cognitive and mpMR/US-fusion biopsies.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Inclusion criteria: PSA &gt; 2.0 ng/ml and/or a positive DRE, and/or a suspicious lesion on TRUS, and PI-RADSv2.1 score ≥ 3. The outcomes evaluated are the detection of clinically significant Pca (ISUP ≥ 2), the overall PСa detection, the detection of clinically insignificant Pca, histological yield (proportion of positive cores, maximum cancer core length).</p></sec><sec><title>Results</title><p>Results. Retrospective data analysis was performed: cognitive biopsy was performed in 102 patients and mpMR/US-fusion biopsy in 176 patients. The median age was 63 years, prostate volume 46 cc. The median PSA was 6.4 ng/ml in the mpMR/US-fusion and 6.7 ng/ml in the cognitive biopsy group. MpMR/US-fusion and cognitive biopsies were comparable about the detection rate of clinically significant (30.3% vs 25.0%; p=0.329) and overall Pca detection rate (50.5% and 42.1%; p = 0.176). It was detected to be less clinically insignificant Pca in the cognitive biopsy group (11.8% vs. 25.5%; p = 0.007).  The proportion of positive cores (30.5% and 29.5% respectively; p = 0.754) and maximum cancer core length (6.6 mm vs 7.6 mm; p = 0.320) were equal when comparing cognitive and mpMR/US-fusion biopsies. The proportion of positive cores with clinically significant Pca was higher in the cognitive biopsy group (18.6% vs 13.1%; p = 0.029).</p></sec><sec><title>Conclusion</title><p>Conclusion. Both cognitive and mpMR/US-fusion biopsies are equally accurate for clinically significant Pca detection.
