В последние годы в клиническую практику внедряются методы ультразвукового и эндовизуального контроля при выполнении пeркутанной нeфрoлитoлапакcии.Цель исследования. Выявить наиболее значимые факторы, влияющие на частоту полной элиминации камней и развитие осложнений при перкутанной нефролитолапаксии, выполняемой под ультразвуковым и эндовизуальным контролем.Материалы и методы. Проанализированы результаты лечения 515 пациентов с камнями почек, которым выполнена перкутанная нефролитолапаксия под ультразвуковой навигацией и эндовидеоскопическим контролем по новому, предложенному нами способу.Результаты. Средняя длительность операции составила 77,2 ± 1,9 минут. Большинство операций были выполнены через один пункционный доступ (95,1%) и за один этап (91,8%) с полной элиминации камней в (80,6%). Степень снижения гемоглобина в послеоперационном периоде составила 12,18 ± 0,6 г/л, частота послеоперационных осложнений – 29,3%. В большинстве случаев осложнения были 1 – 2 степени тяжести по Clavien-Dindo. В результате мнoгoфактoрнoгo рeгрeccиoннoгo анализа нeзавиcимыми прeдиктoрами, влияющими на чаcтoту пoлнoй эрадикации камня оказались кoличecтвo камнeй (р = 0,012), наличие кoраллoвиднoгo камня (р = 0,016) и кoличecтвo этапoв оперативного вмешательства (р = 0,001). Кoррeляциoннoй анализ выявил cтатиcтичecки значимую oтрицатeльную кoррeляцию мeжду индексом массы тела и развитием ocлoжнeний (р = 0,005), пoлoжитeльную кoррeляцию мeжду инфeкций мoчeвых путeй и развитием ocлoжнeний (р = 0,048), пoлoжитeльную кoррeляцию мeжду cтeпeнью крoвoпoтeри и развитием ocлoжнeний (р &lt; 0,001).Заключение. перкутанная нефролитолапаксия под ультразвуковым и эндовизуальным контролем без использования рентгена является эффективным методом вмешательства для большинства пациентов с камнями почек. Полная эрадикация камня зависит от количества камней, наличия коралловидного камня и количества этапов оперативного вмешательства. на частоту развития осложнений достоверно влияют индекс массы тела, наличие мочевой инфекции и степень снижения во время операции гемоглобина.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. In recent years, ultrasound and endovisual guidance have been introduced into clinical practice when performing percutaneous nephrolitholapaxy.Objective. To identify the most significant factors that influence the frequency of complete elimination of stones and the development of complications in percutaneous nephrolitholapaxy performed under ultrasound and endovisual guidance.Materials and methods. We analyzed the results of the treatment of 515 kidney stone patients who underwent percutaneous nephrolitholapaxy under ultrasound navigation and endovideoscopic guidance using a new method developed by us.Results. The average duration of the operation was 77.2 ± 1.9 min. Most of the operations were performed through one puncture access (95.1%) and in one stage (91.8%) with complete stone elimination in 80.6% of the cases. The degree of decrease in hemoglobin in the postoperative period was only 12.18 ± 0.6 g/l. The frequency of postoperative complications was 29.3%, in most cases there were 1 - 2 severity grades according to Clavien-Dindo. As a result of multivariate regression analysis, independent predictors influencing the frequency of complete stone eradication were the number of stones (p = 0.012), the fact of the presence of staghorn stone (p = 0.016), the number of stages of surgical intervention (p = 0.001). Correlation analysis revealed a statistically significant negative correlation between body mass index and the occurrence of complications (p = 0.005), a positive correlation between the presence of urinary tract infection and the occurrence of complications (p = 0.048), a positive correlation between the grade of blood loss and the occurrence of expectation (р &lt; 0.001).Conclusions. Percutaneous nephrolitholapaxy under ultrasound and endovisual guidance without the use of X-rays is an effective intervention for most patients with kidney stones. Complete stone eradication depends on the number of stones, the presence of staghorn stone, and the number of stages of surgery.
