Критериями качества операции являются полное удаление всех участков опухоли в пределах здоровых тканей и, что не менее важно, получение высококачественного гистологического образца с мышечным слоем для постановки точного диагноза. Задача данного обзора — рассмотреть способы увеличения эффективности ТУРМП: 1) возможность улучшения визуализации опухоли в мочевом пузыре; Европейская Ассоциация Урологов рекомендует для этого использовать новые методы — фотодинамическую диагностику (PDD) и узкополосную визуализацию (NBI); 2) резекция опухоли единым блоком (en bloc), что позволяет получить морфологический материал более высокого качества, чем при резекции опухоли по частям, и увеличить безрецидивную выживаемость; 3) использование новых источников энергии (лазеров), что помогает уменьшить риск кровотечений и предотвращает ряд осложнений после ТУРМП, таких как рефлекс запирательного нерва и перфорация мочевого пузыря; а также обобщить последние данные о возможности и необходимости их применения в клинической практике.</p></abstract><trans-abstract xml:lang="en"><p>Transurethral resection of the bladder tumor (TURBT) is a standard procedure in bladder cancer management. TURBT has two main goals: to resect completely all the tumor lesions in healthy tissues and to provide high-quality specimen to facilitate accurate diagnosis. To achieve these goals, urologists make use of several options to maximize the efficiency of the procedure. To make tumor detection as effective as possible, the European Association of Urology guidelines recommend using enhanced visualization methods such as photodynamic diagnostics (PDD) and narrow-band imaging (NBI). Novel en bloc technique enables one to provide specimens of a higher quality and to increase recurrence-free survival. Also, the use of new energy sources such as lasers helps to decrease bleeding and prevent several complications after TURBT, e.g., obturator nerve reflex and bladder perforation.
