В последние годы существенно возрастает интерес к применению радикальной простатэктомии (РПЭ) в качестве одного из компонентов мультимодального подхода у больных лимфогенно-диссеминированным и метастатическим раком предстательной железы (РПЖ). В то же время недостаток крупных рандомизированных исследований не даёт возможности применения данной техники в широкой клинической практике вне рамок клинических исследований.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Оценить эффективность мультмодальной терапии с применением комбинированного химио-гормонального, хирургического и лучевого лечения у больных первичным олигометастатическим гормончувствительным РПЖ.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование включено 48 больных с наличием первичного олигометастатического РПЖ, которые получили комбинированное лечение в рамках внутреннего протокола в одном федеральном исследовательском центре. На первом этапе всем больным проводили комбинированную лекарственную терапию доцетакселом (75 мг/м2 в виде в/в инфузий каждые 3 недели на протяжении 6 курсов) и дегареликсом. Больным, у которых отмечено снижение уровня простатспецифического антигена (ПСА) до уровня ≤ 2 нг/мл и зарегистрирована стабилизация заболевания по данным радиологических методов обследования, проводили хирургическое лечение в объёме радикальной простатэктомии с расширенной тазовой и забрюшинной лимфаденэктомии. Лучевую терапию проводили только больным с наличием костных очагов поражения в дозе 50-70 Гр на зоны проекции костных метастазов в плане 3 этапа комбинированной мультимодальной терапии.</p></sec><sec><title>Результаты</title><p>Результаты. При медиане наблюдения 10 месяцев биохимический рецидив заболевания верифицирован у 27 (56,3%) больных. Среднее время до роста ПСА составило 9,0 ± 5,7 месяцев (от 1 до 24 месяцев), медиана — 7 месяцев. 6-месячная ПСА безрецидивная выживаемость (ПСА-БРВ) составила 61,2 ± 7,5%; 1-летняя ПСА-БРВ — 38,0 ± 8,6%. Средняя продолжительность времени до начала проведения гормональной терапии составила 12 ± 6,1 месяцев (от 3 до 27 месяцев), медиана — 10 месяцев. 6-месячная выживаемость до назначения лекарственного лечения составила 72,6 ± 6,8%; 12-месячная выживаемость — 40,9 ± 8,7%. Около 40% больных олигометастатическим РПЖ не имели признаков прогрессирования заболевания и не получали никакой иной лекарственной терапии на протяжении 12 месяцев после завершения лечения в рамках протокола.</p></sec><sec><title>Заключение</title><p>Заключение. Анализ результатов исследования демонстрирует удовлетворительные онкологические результаты изучаемого варианта терапии у больных с впервые выявленным олигометастатическим гормон-чувствительным РПЖ, а также низкую вероятность развития побочных эффектов и осложнений. Тем не менее, необходимо дальнейшее проведение более крупных и структурированных рандомизированных исследований для определения возможности применения данного терапевтического подхода в клинической практике.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. In recent years, interest in the use of radical prostatectomy (RPE) as one of the components of a multimodal approach in patients with lymphogenous disseminated and metastatic prostate cancer (PCa) has grown significantly. At the same time, the dearth of large randomized trials does not make it possible to use this technique in wide clinical practice outside of clinical trials.</p></sec><sec><title>Purpose of the study</title><p>Purpose of the study. To evaluate the effectiveness of multimodal therapy using combined chemo-hormonal, surgical and radiation therapy in patients with primary oligometastatic hormone-sensitive PCa.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 48 patients with primary oligometastatic prostate cancer who received combination treatment within the internal one-research-center protocol. At the first stage, all patients underwent combined drug therapy with docetaxel (75 mg/m2 intravenously every 3 weeks for 6 courses) and degarelix. Patients who had a decrease in PSA level ≤ 2 ng/ml and registered stabilization of the disease according to radiological examination were treated surgically through RPE with extended pelvic and retroperitoneal lymph node dissection. Radiation therapy was performed only in patients with the presence of bone lesions at a dose of 50-70 Gy to the location of bone metastases in the stage 3 plan of combined multimodal therapy.</p></sec><sec><title>Results</title><p>Results. PCa biochemical relapse was verified in 27 (56.3%) patients during the median follow-up of 10 months. The average time to PSA increase was 9.0 ± 5.7 months (from 1 to 24 months), median — 7 months, Six-month PSA relapse-free survival (PSA-RFS) was 61.2 ± 7.5%; 1-year PSA-RFS — 38.0 ± 8.6%. The average duration before the initiation of hormonal therapy was 12 ± 6.1 months (from 3 to 27 months), median: 10 months. Six-month survival before the drug administration was 72.6 ± 6.8%; twelve-month survival: 40.9 ± 8.7%. About 40% of patients with oligometastatic PCa had no signs of progression and did not receive any other drug therapy for 12 months after completion of protocol treatment.</p></sec><sec><title>Conclusions</title><p>Conclusions. Analysis of the study results demonstrates satisfactory oncological outcomes of the studied treatment option in patients with newly diagnosed oligometastatic hormone-sensitive PCa, as well as a low likelihood of side effects and complications.
