Недержания мочи (НМ) у женщин является актуальной темой среди многих специалистов на сегодняшний день. Доказано, что субуретральная петлевая пластика с использованием синтетических протезов является золотым стандартом в лечении НМ. Начиная с 2005 года идёт неуклонный рост количества оперативных вмешательств с использованием синтетических протезов у данной категории больных, однако, вместе с этим увеличивается и количество осложнений после данной оперативной методики. Методы контроля натяжения синтетической петли до сих пор зависят во многом от опыта хирурга и существующих традиций клиники. В связи с этим мы провели анонимное анкетирование среди специалистов в области урогинекологии, целью которого была оценка основных мнений в диагностике и технике оперативного лечения НМ.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. На кафедре урологии МГМОУ им. А.И. Евдокимова было инициировано сравнительное обсервационное клиническое исследование, в котором использовался анонимный интернет-опрос на базе программы SurveyMonkey, для оценки ответов русскоязычных специалистов урогинекологов по вопросам особенности лечения недержания мочи у женщин. Опрос проводился в период с октября по декабрь 2019 года и состоял из 17 вопросов по различным тематикам. Ответы были проанализированы с использованием описательной статистики.</p></sec><sec><title>Результаты</title><p>Результаты. В анализ были включены ответы 161 специалиста (63% урологов, 37% гинекологов) из 7 стран. Большинство специалистов (47%) имеют небольшой опыт (до 5 лет) хирургического лечения НМ. Заинтересованность в тематике и постоянное прохождение мастер-классов и стажировок отметили 120 респондентов (76,4%). Более 80% специалистов отметили, что используют комплексное уродинамическое исследование в диагностике недержания мочи. почти половина респондентов использовали предоперационные результаты «кашлевой» пробы для определения степени натяжения петли. Также, на вопрос об интраоперационном контроле натяжения синтетического протеза, около половины опрошенных отметили, что используют интраоперационный кашлевой тест, 5,8% применяют маневр Креде и 10,3% используют пробу Вальсальвы. Опрос показал, что специалисты используют различный хирургический инструментарий для контроля натяжения петли интраоперационно.</p></sec><sec><title>Выводы</title><p>Выводы. Наши результаты показывают, что среди специалистов в области урогинекологии существует широкий спектр вариаций в диагностике и технике оперативного лечения НМ. Оценка и разработка методов контроля натяжения синтетической петли, в дальнейшем, является актуальной темой, что потенциально может улучшить качество и результаты лечения пациенток с НМ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Urinary incontinence (UI) in women is a hot topic among many professionals today. It is proved that mid-urethral synthetic tape is the gold standard in the treatment of UI. There has been a steady increase in the number of operations using synthetic prostheses in this category of patients since 2005. However, along with this, the number of complications after this surgical technique is also increasing. Methods for controlling the tension of the synthetic tape are still largely dependent on the experience of the surgeon and the existing traditions of the clinic.</p><p>In this regard, we conducted an anonymous survey among specialists of urogynecologists, where we found out the main opinions in the diagnosis and technique of surgical treatment of UI.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Department of Urology, A.I. Yevdokimov Moscow State University of Medicine and Dentistry has initiated a comparative observational clinical trial to assess the responses of Russian-speaking specialists of urogynecologists on the specifics of treatment of urinary incontinence in women using an anonymous online survey based on SurveyMonkey. The survey was conducted from October to December 2019 and consisted of 17 questions on various topics. Responses were analyzed using descriptive statistics.</p></sec><sec><title>Results</title><p>Results. The analysis included the answers of 161 specialists (63% of urologists, 37% of gynaecologists) from 7 countries. Most specialists (47%) have little experience (up to 5 years) of surgical treatment of UI. Interest in topics and masterclasses and internships was noted by 120 respondents (76.4%). More than 80% of specialists noted that they use a urodynamic study in the diagnosis of UI. Half of the respondents used the preoperative "cough" test results to determine the tape tension. About half of the respondents noted that they use the intraoperative "cough" test, 5.8% use the Crede manoeuvre, and 10.3% use the Valsalva test. The survey showed that specialists use various surgical instruments to control the tape tension intraoperatively.</p></sec><sec><title>Conclusions</title><p>Conclusions. Our results show that among specialists in urogynecology there is a wide range of variations in the diagnosis and technique of surgical treatment of UI.
