Повышенный индекс массы тела (ИМТ) может являться одним из неблагоприятных прогностических факторов рака предстательной железы (РПЖ), связанных с худшими онкологическими и функциональными результатами радикальной простатэктомии (РПЭ).</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Оценить патоморфологические характеристики, безрецидивную выживаемость и восстановление эректильной функции (ЭФ) после РПЭ с нервосберегающей техникой (НСТ) у пациентов с ожирением.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Группу исследования составил 91 пациент с ИМТ ≥ 30 кг/м2 , контрольную группу — 356 пациентов с ИМТ &lt; 30 кг/м2 , перенесших РПЭ с односторонней или двухсторонней НСТ в период с января 2012 г. по декабрь 2019 г. Выполнен сравнительный анализ патоморфологических результатов, частоты осложнений, безрецидивной выживаемости, динамики восстановления ЭФ в обеих группах.</p></sec><sec><title>Результаты</title><p>Результаты. Пациенты с ожирением имели больший объём предстательной железы, большую сумму баллов по опроснику Международной системы суммарной оценки заболеваний простаты (IPSS). Одно- и двухсторонняя НСТ использовались в обеих группах в равных пропорциях: 50,5 % и 49,5 % в группе с ИМТ ≥ 30 и 51,4 % и 48,6 % в группе с ИМТ &lt; 30 (p = 0,88). Между группами не выявлено значимых различий в частоте неблагоприятных патоморфологических характеристик, серьезных послеоперационных осложнений и объёме интраоперационной кровопотери. Пятилетняя безрецидивная выживаемость после РПЭ составила 93,1 % в группе ИМТ ≥ 30 и 95,1 % в группе ИМТ &lt; 30 (p = 0,55). Общая частота восстановления ЭФ после РПЭ с НСТ через 24 месяца составила 75 % и 78,5 % (p = 0,24). При этом восстановление ЭФ у пациентов с ожирением происходило медленнее: достаточная для полового акта ЭФ через 6 и 12 месяцев наблюдалась у 17,9 % и 32,1 % против 35,4 % и 53,8 % в группе с ИМТ &lt; 30, а среднее временя до восстановления составило 10,9 (±1) и 8,6 (±0,6) месяцев соответственно (p = 0,04).</p></sec><sec><title>Выводы</title><p>Выводы. Ожирение не влияет на патоморфологические и онкологические результаты РПЭ с НСТ. Восстановление ЭФ у пациентов с ИМТ ≥ 30 замедлено, однако через 24 месяца после операции результаты сравнимы с уровнем ЭФ у пациентов с ИМТ &lt; 30. Полученные данные могут иметь значение при консультировании пациентов и планировании оперативного вмешательства у больных РПЖ с ожирением.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. According to several studies, an increased body mass index (BMI) may be one of the unfavorable prognostic factors of prostate cancer (PC) associated with lower oncological and functional outcomes of radical prostatectomy (RP).</p></sec><sec><title>Purpose of the study</title><p>Purpose of the study. To evaluate pathomorphological characteristics, recurrence-free survival, and restoration of erectile function (EF) after RP with nerve-sparing technique (NST) in obese patients.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study group consisted of 91 patients with BMI ≥ 30 kg/m2 , the control group consisted of 356 patients with BMI &lt; 30 kg/m2 who underwent RP with unilateral or bilateral NST from January 2012 to December 2019. A comparative analysis of pathomorphological results, the rate of complications, recurrence-free survival, and the dynamics of EF restoration in both groups was performed.</p></sec><sec><title>Results</title><p>Results. Obese patients had a larger prostate volume, a higher score for the International Prostate Symptom Score (IPSS) questionnaire. Unilateral and bilateral NST was used in both groups in equal proportions: 50.5% and 49.5% in the group with BMI ≥ 30 and 51.4% and 48.6% in the group with BMI &lt; 30 (p = 0.88 ) There were no significant differences between the groups in the rate of adverse pathomorphological characteristics, serious postoperative complications and the volume of intraoperative blood loss. The five-year recurrence-free survival after RP was 93.1% in the BMI group ≥ 30 and 95.1% in the BMI group &lt; 30 (p = 0.55). The total rate of EF recovery after RP with NST after 24 months was 75% and 78.5% (p = 0.24). The restoration of EF in obese patients was slower: sufficient for sexual intercourse EF after 6 and 12 months was observed in 17.9% and 32.1% versus 35.4% and 53.8% in the group with BMI &lt; 30, and the meantime to recovery was 10.9 (± 1) and 8.6 (± 0.6) months, respectively (p = 0.04).</p></sec><sec><title>Conclusions</title><p>Conclusions. Obesity does not affect the pathomorphological and oncological results of RP with NST. EF recovery in patients with a BMI of ≥ 30 is slowed down, however, 24 months after surgery, the results are comparable with the potency level in patients with a BMI &lt; 30.
