Доброкачественная гиперплазия предстательной железы (ДГПЖ) является одним из самых распространённых заболеваний среди мужчин. Хирургическое вмешательство рекомендовано в случае нежелания пациента начинать или продолжать медикаментозное лечение или в случае прогрессирования симптомов нижних мочевых путей (СНМП), или осложнённого течения ДГПЖ. За последние два десятилетия наблюдается значительное расширение спектра видов хирургического лечения.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Проведение сравнительного анализа эндовидеохирургических методов лечения ДГПЖ.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Для написания обзора по данной проблематике были проанализированы следующие базы данных: PubMed, ScienceDirect, Cohrane Library, UpToDate, eLibrary с использованием ключевых слов: «benign prostatic hyperplasia», «urethral stricture», «bladder neck contracture», «transurethral resection of prostate», «holmium laser», «thulium laser», «GreenLight laser». Диапазон поиска: с 1989 по 2019 года.</p></sec><sec><title>Результаты</title><p>Результаты. Исходя из результатов многочисленных исследований тулиевый, гольмиевый и Greenlight лазеры показывают наилучшие результаты. Операции с использованием лазера превосходят по длительности трансуретральную резекцию простаты (ТУРП), однако показатели времени госпитализации и катетеризации имеют большую значимость для пациентов. Эффективность и безопасность также являются двумя важнейшими критериями и в этом плане тулиевый и гольмиевый лазеры отлично себя показывают. Тулиевый лазер обеспечивает отличный результат вапоризации ткани предстательной железы и надёжный гемостаз при этом не нанося значительную термическую травму окружающим тканям. Для гольмиевого лазера характерны наименьшие показатели рекатетеризации, задержки мочи, воспалительно-инфекционные осложнения, стрессового недержания мочи и ретроградной эякуляции, однако стоит отметить вероятность повреждения капсулы предстательной железы. Greenlight из-за специфики длины его волны обладает прекрасным гемостазом, однако это влечёт за собой большее термическое повреждение окружающих тканей.</p></sec><sec><title>Выводы</title><p>Выводы. Безусловно все рассмотренные техники являются достаточно безопасными и эффективным, но ввиду особенностей каждого из методов, для улучшения эффекта хирургического лечения необходимо грамотная селекция метода вмешательства для каждого пациента.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Benign prostatic hyperplasia (BPH) is one of the commonest diseases in male population. Surgical treatment is recommended in cases if a patient doesn't want to begin or continue his medication or lower urinary tract symptoms (LUTS) progression or complicated course of the disease. Over the past two decades there has been a significant expansion of the range of types of surgical treatment.</p></sec><sec><title>Objectives</title><p>Objectives. Conduct a comparative analysis of endovideosurgical treatment of BPH.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. In the course of work on this issue, the following databases were analyzed: PubMed, ScienceDirect, Cohrane Library, UpToDate, eLibrary with a search range in the last 30 years, next key words were used: «benign prostatic hyperplasia», «urethral stricture», «bladder neck contracture», «TuRP», «HoLEP», «ThuLEP», «GreenLight laser». Search range: from 1989 to 2019.</p></sec><sec><title>Results</title><p>Results. Based on the results of numerous studies, thulium, holmium and GreenLight lasers show the best results. Laser surgery is longer than the transurethral resection of the prostate (TURP), however, the rates of hospitalization and catheterization are more significant for patients. Efficiency and safety are also two of the most important criteria, and in this respect, thulium and holmium lasers perform well. Thulium laser provides an excellent result of vaporization of the prostate tissue and reliable hemostasis without causing significant thermal injury to the surrounding tissues. Holmium laser shows the lowest rates of re-catheterization, urinary retention, inflammatory and infectious complications, stress urinary incontinence and retrograde ejaculation, but there is a chance of damaging the prostate gland capsule.
