Основным методом оперативного лечения больных с мышечно-инвазивным раком мочевого пузыря (РМП) остаётся радикальная цистэктомия (РЦ). За последние годы при выполнении РЦ наряду с открытым используются лапароскопический и робот-ассистированный доступы.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Описать этапы и интраоперационные результаты робот-ассистированной РЦ.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. С июня по ноябрь 2018 года робот-ассистированная РЦ с интракорпоральной ортотопической цистопластикой выполнена 10 пациентам (8 мужчин, 2 женщины), возраст — 64-76 лет. Индекс массы тела составил 25,6±4,5 кг/м2. Предоперационное обследование включало у льтразвуковое исследование (УЗИ) и спиральную компьютерную томографию (СКТ) органов брюшной полости и малого таза, СКТ лёгких, лабораторные исследования. Операцию выполняли в положении Тренделенбурга на спине. При РЦ выделяли следующие этапы: выделение мочеточников в нижней трети, заднюю диссекцию мочевого пузыря (МП), мобилизацию МП с обеих сторон до эндопельвикальной фасции, клипирование и пересечение пузырных сосудов, прошивание дорзального венозного комплекса и отсечение уретры.</p></sec><sec><title>Результаты</title><p>Результаты. Конверсии в открытое вмешательство не было. Время РЦ (от установки троакаров до удаления МП) — от 100 до 240 мин, среднее время составило 120 мин. Объём кровопотери — от 250 до 800 мл (средний 370 мл). Основная кровопотеря отмечалась на этапе мобилизации простаты и дорзального венозного комплекса. Гемотрансфузия потребовалась 3 пациентам. При патоморфологическом исследовании рТ2 стадия выявлена у 6, рТ3 — у 4 больных. У 3 пациентов также выявлена аденокарцинома простаты. Удалённые лимфоузлы были негативными во всех случаях.</p></sec><sec><title>Заключение</title><p>Заключение. Робот-ассистированная РЦ является современным малоинвазивным методом лечения больных с мышечно-инвазивным РМП. Поэтапный подход при её выполнении позволяет уменьшить время операции и количество послеоперационных осложнений.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The main method of treating patients with muscle invasive bladder cancer remains radical cystectomy (RC). In recent years in RC along with open access are used laparoscopic and robot-assisted approaches.</p></sec><sec><title>Purpose of research</title><p>Purpose of research. Describe main steps of robot-assisted RC.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. From June 2018 to November 2019 10 patients were underwent robot-assisted RC with intracorporeal orthotopic ileocystoplasty. There were 8 male, and 2 female. Age of patents ranged from 54 to 76 years. Body mass index was 25.6±4.5 kg/м2. Preoperative examination included USI and CT of abdomen and pelvis, lchest CT, laboratory analyses. Procedure performed in Tredelenburg position. RC included next steps: mobilization of distal part of ureters, posterior dissection of bladder, lateral dissection of the bladder, vesicle pedicle is clipped by Hem-o-Lok clips, dorsal venous complex stitch and dissection of the urethra.</p></sec><sec><title>Results</title><p>Results. No conversion to open surgeries. Operative time of RC ranged from 100 to 240 min (mean – 120 min). Blood loss volume ranged from 259 ml to 800 ml (mean 370 ml), and generally blood loss was noted during mobilization of a prostate and a dorsal venous complex. The haemotransfusion was carried out to 3 patients. Morphologic examination is revealed T2 stage in 6 patients, T3 in 4 patients. Three patients also had adenocarcinoma of prostate. Lymph nodes were negative in all patients.</p></sec><sec><title>Conclusion</title><p>Conclusion. Robot-assisted RC is a mini-invasive method for treatment of patients with muscle-invasive bladder cancer.
