Удвоение почки является одной из наиболее частых аномалий мочевыводящей системы. Подходы к оперативному лечению данных состояний значимо отличаются. В зависимости от предпочтений хирургов предлагается проводить геминефруретерэктомию выполняемую открытым или лапароскопическим доступом, уретероцистоанастомоза эктопированного мочеточника или двух мочеточников единым блоком. В последнее время стали появляется публикации о возможности наложения межмочеточниковых анастомозов. Мы представляем опыт проведения открытого уретероуретероанастомоза (УУА) и лапароскопического уретероуретероанастомоза (ЛУУА) в 9 клиниках России и Белоруссии.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Улучшение результатов лечения детей с уретерогидронефрозом половины удвоенной почки.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Ретроспективному анализу подвергнуты истории болезни 38 детей, оперированных с 2007 по 2016 гг. (10 мальчиков (26,3%), 28 девочек (73,7%)). Средний возраст пациентов на момент проведения операции составил 40,2 месяца. Патология удвоенной правой почки определена у 15 (39,5%) детей, левой — у 23 (60,5%). В зависимости от применяемого оперативного пособия все пациенты были разделены на две группы. В первую группу включены 14 (36,8%) детей, у которых уретероуретероанастомоз наложен открытым доступом, во вторую — 24 (63,2%) пациентов с наложением анастомоза лапароскопическим доступом.</p></sec><sec><title>Результаты</title><p> Результаты. Конверсий в данной серии пациентов не было. Обострение пиелонефрита после операции возникло по 1 случаю в каждой группе. В раннем послеоперационном периоде подтекание мочи по дренажу отмечено у 3 (12,5%) пациентов после наложения ЛУУА. У 1 (4,2%) ребенка развился культит. Таким образом, осложнения наблюдались у 5 (13,2%) детей, при этом повторное вмешательство потребовалось 1 (2,6%) пациенту.</p></sec><sec><title>Заключение</title><p>Заключение. УУА, ЛУУА при лечении патологических состояний удвоенной почки выполняются немногочисленными клиниками и находятся в стадии развития и накопления опыта. Проведение УУА или ЛУУА является безопасным методом лечения, позволяющий минимизировать риск потери функции удвоенной почки.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The Duplex kidney is one of the most common anomalies of urinary system. Approaches to the surgical management of these conditions significantly differ. Taking into account the preferences of surgeons, heminephroureterectomy is offered to perform openly or laparoscopically, uretero-cysto-anastomosis of ectopic ureter or two ureters en bloc. Some publications about the possibility of ureteroureteral anastomosis have lately begun to appear. We present the experience of ureteroureteral anastomosis performed openly and laparoscopically in 9 clinics of Russia and Belarus.</p></sec><sec><title>Objective</title><p>Objective. Improvement of treatment results of ureterohydronephrosis in children with complete ureteral duplication.</p></sec><sec><title>Material and methods</title><p>Material and methods. We retrospectively reviewed medical records of 38 children who underwent ureteroureteral anastomosis surgery from 2007 to 2016. There were 10 boys (26.3%) and 28 girls (73.7%). The mean age of patients was 40.2 months. Right duplex kidney was confirmed in 15 (39.5%) children, the left one was diagnosed in 23 (60.5%) children. All the patients were divided into two groups according to the surgical technique used. Fourteen children 14 (36.8%) were included in the first group, in whom the ureteroureteral anastomosis has been performed openly, the second group comprised 24 (63.2%) patients with laparoscopically made anastomosis.</p></sec><sec><title>Results</title><p>Results. There weren’t any cases of conversion in this cohort. One patient in each group suffered from acute pyelonephritis. In the early postoperative period, urine leakage along the drainage occurred in 3 (12.5%) patients after laparoscopically performed ureteroureteral anastomosis, one (4.2%) child had residual ureteral stump. Thus, five (13.2%) children had complications, one (2.6%) patient required redo surgery.</p></sec><sec><title>Conclusion</title><p>Conclusion. There are few clinics where ureteroureteral anastomosis is performed via open or laparoscopic approach. This operation is at the stage of development and accumulation of experience.
