Радикальная цистэктомия (РЦЭ) обеспечивает эффективный контроль рака мочевого пузыря, но связана с общим уровнем осложнений в течение 90 дней &gt; 50%, уровнем серьёзных осложнений 15 – 20% и риском смертности 2%. Осложнения, связанные с РЦЭ, развиваются реже, а большинство осложнений, наблюдаемых в краткосрочной и долгосрочной перспективе после операции, связаны с использованным методом деривации мочи.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Оценить количество и структуры отсроченных послеоперационных осложнений у пацентов после РЦЭ в зависимости от методики уродеривации при немышечно-инвазивном раке и раке мочевого пузыря минимальной интрадетрузорной инвазии.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование включены пациенты с немышечно-инвазивным раком мочевого пузыря (НМИРМП) высокого риска прогрессии — 49 больных (группа 1) и пациенты с мышечно-инвазивным раком мочевого пузыря (МИРМП) с минимальной интрадутрузорной инвазией — 102 больных (группа 2).</p></sec><sec><title>Результаты</title><p>Результаты. Отсроченные осложнения по распространённости случаев на 100 больных выявлены в 10,9 ± 4,9 против 12,1 ± 3,5 случаев на 100 больных в группах 1 и 2 соответственно, без достоверной разницы. Частота случаев отсроченных осложнений, связанных с уродеривацией, была почти в 5,5 раз выше, чем по группе осложнений, не связанных с уродеривацией. Вне зависимости от практически одинаковых долей пациентов с осложнениями, связанных с уродеривацией (39,1% и 39,4% в группах 1 и 2 соответственно), общая распространённость случаев этих осложнений была большей в группе 2 (71,7 ± 8,5 против 60,9 ± 11,5 случаев на 100 больных в группе 1; р &gt; 0,05). Внутренний способ отведения мочи был связан с повышенным риском развития осложнений (266,7 ± 66,7 случаев на 100 пациентов) в сравнении с другими видами уродеривации. Кроме того, наиболее благоприятной методикой уродеривации в отношении развития отсроченных осложнений оказалась инвертированная илеоцистопластика (34,8 ± 12,3 случая на 100 пациентов). Вместе с тем самая низкая частота коррекционных операций выявлена в группе пациентов с уродеривацией по Bricker (6,3 ± 4,4 случая на 100 пациентов).</p></sec><sec><title>Заключение</title><p>Заключение. В отсроченном периоде наблюдения выявлятся более высокая частота осложнений, связанных с уродеривацией, и специфических урологических осложнений у пациентов группы 2, но повышенная частота коррекционных операций, связанных с уродеривацией, — у пациентов 1-й группы. При учёте способов уродеривации определено неблагополучие пациентов с внутренним способом отведения мочи и наиболее «благополучные» при ортотопической инфертированной илеоцистопластике.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Radical cystectomy (RC) effectively controls bladder cancer but carries a &gt; 50% 90-day complication rate, 15 – 20% severe complications, and 2% mortality. Most short- and long-term complications relate to urinary diversion rather than RC itself.</p></sec><sec><title>Objective</title><p>Objective. To evaluate the incidence and profile of delayed postoperative complications following RC by urinary diversion method in non-muscle-invasive bladder cancer (NMIBC) and minimal intradetrusor-invasive muscle-invasive bladder cancer (MIBC).</p></sec><sec><title>Materials &amp; methods</title><p>Materials &amp; methods. This study included 49 patients with high-risk progressive NMIBC (group 1) and 102 with MIBC exhibiting minimal intradetrusor invasion (group 2) who underwent RC.</p></sec><sec><title>Results</title><p>Results. Delayed complications occurred in 10.9 ± 4.9 (group 1) versus 12.1 ± 3.5 cases per 100 patients (group 2; no significant difference). Diversion-related complications were 5.5 times more frequent than non-diversion-related ones. Proportions of affected patients were similar (39.1% vs 39.4%), but case prevalence was higher in group 2 (71.7 ± 8.5 vs 60.9 ± 11.5 per 100; p &gt; 0.05). Internal diversion showed the highest risk (266.7 ± 66.7 per 100), while inverted ileocystoplasty was most favourable (34.8 ± 12.3 per 100). Bricker diversion had the lowest corrective surgery rate (6.3 ± 4.4 per 100).</p></sec><sec><title>Conclusion</title><p>Conclusion. Delayed diversion-related and urological complications were more prevalent in minimal MIBC, with higher corrective surgeries in NMIBC.
