Рак предстательной железы остаётся ведущим онкологическим заболеванием у мужчин, требующим точной диагностики. Современные методы, такие как мультипараметрическая МРТ (мпМРТ) со шкалой PI-RADS и таргетированная биопсия, позволяют повысить обнаружение клинически значимых опухолей, снизить ненужные вмешательства и минимизировать осложнения.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Оценить эффективность трансректальной мпМРТ / УЗИ fusion биопсии предстательной железы для диагностики клинически значимого рака простаты.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Трансректальная мпМРТ / УЗИ fusion биопсия простаты выполнена 318 пациентам. Всем пациентам проводили предварительную мпМРТ с оценкой PI-RADS, определяли плотность ПСА, выполняли пальцевое ректальное исследование (ПРИ). Биопсию выполняли комбинированно: таргетная (под мпМРТ / УЗ-фьюжн навигацией) по выявленным очагам, а также систематическая. Оценивали частоту выявления рака простаты (в том числе клинически значимого, ISUP ≥ 2) и её связь с PI-RADS, плотностью ПСА, данными ПРИ; сравнивали результаты первичных и повторных биопсий. Анализировали профиль безопасности.</p></sec><sec><title>Результаты</title><p>Результаты. Клинически значимый рак простаты (ISUP ≥ 2) выявлен у 40% пациентов; при первичной биопсии — у 43%, при повторной — у 35% (p &gt; 0,05). Частота выявления зависела от категории PI-RADS: при PI-RADS 4 – 5 значимый рак обнаруживался значительно чаще, чем при PI-RADS 3. У пациентов с PI-RADS 3 плотность ПСА &gt; 0,15 нг/мл/см³ ассоциирована с более высокой вероятностью выявления рака, тогда как низкая плотность ПСА соответствует низкому риску. Положительное ПРИ также коррелировало с наличием опухоли. Таргетная биопсия выявила большинство опухолей, однако в сочетании с систематической биопсией увеличилась общая выявляемость клинически значимого рака (дополнительно 10% случаев). Серьёзных осложнений (сепсис, выраженное кровотечение) не отмечено; незначительные побочные эффекты (кратковременная гематурия, лихорадка) имели место менее чем у 5% пациентов.</p></sec><sec><title>Заключение</title><p>Заключение. Трансректальная мпМРТ / УЗИ fusion биопсия предстательной железы обладает высокой эффективностью в выявлении клинически значимого рака простаты. Полученные результаты подтверждают важность предварительного выполнения мпМРТ и комбинированного (таргетного и систематического) подхода к биопсии для оптимизации диагностики клинически значимого рака при минимальном риске осложнений.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Prostate cancer remains one of the leading oncological diseases among men, necessitating precise diagnostic strategies. Contemporary approaches, including multiparametric MRI (mpMRI) with PI-RADS scoring and targeted biopsy, enhance detection of clinically significant prostate cancer (csPCa), reduce unnecessary procedures, and minimize complications.</p></sec><sec><title>Objective</title><p>Objective. To evaluate the efficacy of transrectal mpMRI/transrectal ultrasound (TRUS) fusion-guided prostate biopsy in detecting csPCa.</p></sec><sec><title>Materials &amp; methods</title><p>Materials &amp; methods. We prospectively evaluated 318 patients who underwent transrectal mpMRI/TRUS fusion-guided prostate biopsy. All patients had pre-biopsy mpMRI with PI-RADS scoring, prostate-specific antigen (PSA) levels, PSA density, and digital rectal examination (DRE). Biopsies employed a combined approach: targeted sampling of mpMRIidentified lesions plus systematic sampling. Prostate cancer detection rates (csPCa defined as ISUP grade ≥ 2) were correlated with PI-RADS score, PSA density, and DRE findings. Detection rates were compared between biopsy-naïve and repeat-biopsy patients. Complications were systematically recorded.</p></sec><sec><title>Results</title><p>Results. csPCa was detected in 40% of patients overall (43% in biopsy-naïve vs. 35% in repeat-biopsy patients; p &gt; 0.05). Lesions scored PI-RADS 4 – 5 yielded significantly higher csPCa detection rates than PI-RADS 3 lesions. Among PI-RADS 3 cases, PSA density &gt; 0.15 ng/mL/cm³ predicted higher csPCa risk, while lower values indicated low risk. Positive DRE also correlated with csPCa detection. Targeted biopsy detected most csPCa cases, but systematic sampling increased the overall yield by 10%. No severe complications (e.g., sepsis or major hemorrhage) occurred; minor, transient events such as hematuria or fever affected &lt; 5% of patients.</p></sec><sec><title>Conclusions</title><p>Conclusions. Transrectal mpMRI/TRUS fusion-guided biopsy is highly effective for csPCa detection.
