В структуре общего травматизма повреждения почек составляют от 2 до 10% случаев. Вопрос лечения разрыва почки, проникающего в чашечно-лоханочную систему (ЧЛС), является дискутабельным. В последнее десятилетие появляются данные, подтверждающие высокую эффективность консервативного метода лечения в соответствии с протоколами non-operative management (NOM).</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Определение оптимальной тактики лечения закрытой травмы почки, проникающей в чашечно-лоханочную систему.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование включены 43 пациента, находившихся на стационарном лечении в Научно-исследовательском институте скорой помощи им. И.И. Джанелидзе с 1998 года по 2023 год, с закрытой травмой почки, проникающей в ЧЛС. Пациенты были разделены на две группы – проспективную и ретроспективную. Тяжесть повреждений почки оценивали по шкале Американской ассоциации хирургической травмы (AAST). В соответствии с балльной оценкой Ю.Н. Цибина проводили оценку тяжести травматического шока. Лечение травм почек, проникающих в ЧЛС, проводили консервативным и хирургическим способами.</p></sec><sec><title>Результаты</title><p>Результаты. Медиана возраста у пациентов составила 43 [35 – 55] года. Соотношение мужчин и женщин составило 32:11. Лечение большинства пациентов из проспективной группы проводили консервативным (n = 12) и малоинвазивным (n = 9) способами. В ретроспективной группе всем пострадавшим (n = 21) выполняли открытые хирургические вмешательства. В ретроспективной группе отмечалась более высокая частота выполнения нефрэктомий. Частота развития осложнений и летальность у пациентов разных групп статистически достоверно не различались.</p></sec><sec><title>Заключение</title><p>Заключение. Консервативный способ является оптимальным при лечении закрытой травмы почки, проникающей в ЧЛС. В случаях развития мочевых затёков следует выполнять стентирование мочеточника или пункционную нефростомию.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Renal trauma accounts for 2–10% of all injuries to the urinary system. The optimal management strategy for renal trauma with collecting system rupture remains controversial. Over the past decade, accumulating evidence has demonstrated the high efficacy of non‑operative management (NOM) in this setting.</p></sec><sec><title>Objective</title><p>Objective. To determine the optimal management approach for blunt renal trauma with collecting system rupture.</p></sec><sec><title>Materials &amp; methods</title><p>Materials &amp; methods. From 1998 to 2023, 43 patients with blunt renal trauma complicated by collecting system rupture were treated at the Dzhanelidze Research Institute of Emergency Medicine. The severity of renal injury was graded according to the American Association for the Surgery of Trauma (AAST) classification. The severity of traumatic shock was assessed using the Y.N. Tsibin scoring system. Patients were divided into two groups. Management strategies for blunt renal trauma with collecting system rupture included both conservative and surgical approaches.</p></sec><sec><title>Results</title><p>Results. The median age of the patients was 43 years (interquartile range 35 – 55), and the male-to-female ratio was 32:11. In the prospective group, most patients received conservative (n = 12) or minimally invasive (n = 9) treatment, whereas in the retrospective group all patients (n = 21) underwent open surgery. The nephrectomy rate was higher in the retrospective group. There were no statistically significant differences between the groups in terms of complication rates or length of hospital stay.</p></sec><sec><title>Conclusion</title><p>Conclusion. Conservative management is the preferred approach for blunt renal trauma with collecting system rupture.
