Сочетанная хирургическая патология создаёт дилемму выбора между симультанным и этапным подходами. Симультанные операции, сокращают время госпитализации, снижают анестезиологическую нагрузку и уменьшают затраты, но увеличение объёма и продолжительности вмешательства может повысить риск осложнений. Этапный подход снижает одномоментную нагрузку на пациента, но требует повторных госпитализаций, увеличивает общее время лечения и реабилитации, а также затраты на медицинское обслуживание.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Сравнить безопасность и эффективность симультанных и этапных операций в рамках концепции программы ускоренного выздоровления (ERAS) при проведении планового хирургического лечения в урологии.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В проспективное одноцентровое рандомизированное исследование, посвящённое оценке безопасности и эффективности симультанных и этапных операций в урологии, были включены 78 пациентов с сочетанием урологической и хирургической патологий, рандомизированных в две группы. Группу 1 (n = 53) подвергали симультанным операциям, при которых несколько хирургических вмешательств проводили одномоментно. Группа 2 (n = 25) прошла этапное оперативное лечение, разделённое во времени. В обеих группах соблюдение протокола ускоренного выздоровления более чем на 80% являлось критерием для включения. Оценивали периоперационные показатели, включая продолжительность хирургического вмешательства, размер суммарного хирургического доступа, уровень интраоперационной кровопотери, частоту осложнений по классификации Clavien-Dindo, послеоперационный болевой синдром, согласно ВАШ, необходимость в наркотических анальгетиках, общее количество дней дренирования мочевых путей, продолжительность госпитализации.</p></sec><sec><title>Результаты</title><p>Результаты. Симультанные операции продемонстрировали значительное сокращение общего времени госпитализации (p &lt; 0,01) и продолжительности суммарного времени оперативного лечения по сравнению с этапными вмешательствами. При этом частота послеоперационных осложнений не имела статистически значимых различий между группами (p = 0,94).</p></sec><sec><title>Заключение</title><p>Заключение. Симультанные операции в урологии являются безопасным и эффективным подходом при лечении пациентов с сочетанной патологией и обеспечивают преимущества в виде сокращения времени госпитализации и снижения экономических затрат без увеличения риска послеоперационных осложнений.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Combined surgical pathology poses a dilemma between simultaneous and staged approaches. Simultaneous surgeries, performed during a single anesthetic session, reduce hospitalization time, lower anesthetic burden, however, increased operative volume and duration may elevate the risk of complications. The staged approach reduces the immediate burden on the patient but requires multiple hospitalizations, prolongs overall treatment and recovery time.</p></sec><sec><title>Objective</title><p>Objective. To compare clinical outcomes, length of hospitalization, postoperative complications and effectiveness of simultaneous versus staged urological surgeries within a structured enhanced recovery after surgery (ERAS) protocol.</p></sec><sec><title>Materials &amp; methods</title><p>Materials &amp; methods. A prospective single-center randomized study evaluating the safety of simultaneous and staged urological surgeries included 78 patients with combined urological and surgical pathologies, randomized into two groups. Compliance with the ERAS protocol above 80% was a criterion for inclusion in both groups. Group 1 (n = 53) underwent simultaneous surgeries, where multiple procedures were performed during a single operative session. Group 2 (n = 25) received staged treatment, with surgical interventions separated in time. Perioperative parameters, complication rates according to the Clavien-Dindo classification, length of hospitalization, patient-reported outcomes including postoperative pain assessed by the visual analog scale (VAS), requirement for opioid analgesics, intraoperative blood loss, total duration of urinary drainage, and cumulative surgical access size were evaluated.</p></sec><sec><title>Results</title><p>Results. Simultaneous surgeries demonstrated a significant reduction in overall hospitalization time (p &lt; 0.01) and total operative time compared with staged interventions. No statistically significant differences in postoperative complication rates were observed between groups (p = 0.94).</p></sec><sec><title>Conclusion</title><p>Conclusion. Both simultaneous and staged urological surgeries are safe approaches for patients with combined pathology.
