Подавляющая часть опухолей мочевого пузыря представлена переходно-клеточной карциномой, тогда как доля лейомиом не превышает 0,5%. Исключительная редкость этой мезенхимальной опухоли обусловливает трудности в диагностике и отсутствие единых алгоритмов лечения. Тактика лечения зависит от локализации и размеров образования.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Описать технику оперативного лечения пациентки с лейомиомой мочевого пузыря и особенности клинического течения данного заболевания.</p></sec><sec><title>Презентация случая</title><p>Презентация случая. Представлено клиническое наблюдение женщины 65 лет с лейомиомой мочевого пузыря. Для диагностики применяли ультразвуковое исследование, магнитно-резонансную томографию с контрастированием и цитологическое исследование мочи. Больной выполнена трансуретральная лазерная энуклеорезекция тулиевым волоконным лазером U-max. Длительность операции — 35 минут, интраоперационных осложнений не наблюдали. В раннем послеоперационном периоде после удаления уретрального катетера развились нарушения мочеиспускания, субфебрильная лихорадка и болевой синдром, что потребовало повторной катетеризации мочевого пузыря. При гистологическом исследовании установлен диагноз «Лейомиома мочевого пузыря».</p></sec><sec><title>Клиническое обсуждение</title><p>Клиническое обсуждение. Лейомиома мочевого пузыря представляет собой доброкачественное новообразование из группы мезенхимальных опухолей. Отсутствие стандартов лечения лейомиомы мочевого пузыря диктует необходимость индивидуального подхода к лечению заболевания. Описаны различные варианты оперативного лечения, но основным методом остаётся трансуретральная резекция. Одним из преимуществ эндоскопического вмешательства является малоинвазивный характер, короткий срок госпитализации и период реабилитации. При наличии данных за выраженный внепузырный рост образования предпочтительна открытая или лапароскопическая резекция ввиду риска интраоперационной перфорации при эндоскопии.</p></sec><sec><title>Заключение</title><p>Заключение. Клинический случай подтверждает, что трансуретральная лазерная энуклеорезекция является эффективным и безопасным методом лечения лейомиомы мочевого пузыря, обеспечивающим радикальное удаление при минимальной травматичности. Раннее применение комплексной диагностики и индивидуализированный выбор оперативного лечения обеспечивают благоприятный прогноз при данной редкой патологии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Most bladder tumors are represented by transitional cell carcinoma, while leiomyomas account for less than 0.5% of cases. The extreme rarity of this mesenchymal tumor leads to diagnostic challenges and the absence of unified treatment protocols. The management strategy depends on the location and size of the lesion.</p></sec><sec><title>Objective</title><p>Objective. To describe the surgical technique and clinical features of bladder leiomyoma in a female patient.</p></sec><sec><title>Case presentation</title><p>Case presentation. We report a clinical case of a 65-year-old woman diagnosed with bladder leiomyoma. Ultrasonography, contrast-enhanced magnetic resonance imaging, and urine cytology were performed for diagnosis. The patient underwent transurethral laser enucleoresection using a U-max thulium fiber laser. The operation lasted 35 minutes without intraoperative complications. In the early postoperative period, following the removal of the urethral catheter, the patient developed voiding difficulties, subfebrile fever, and pelvic pain syndrome, requiring repeat bladder catheterization. Histological examination confirmed the diagnosis of bladder leiomyoma.</p></sec><sec><title>Clinical discussion</title><p>Clinical discussion. Bladder leiomyoma is a benign mesenchymal tumor. The absence of standardized treatment guidelines necessitates an individualized therapeutic approach. Various surgical options have been described, but transurethral resection remains the primary method. Endoscopic intervention is minimally invasive, provides shorter hospital stays, and promotes rapid rehabilitation. In cases of pronounced extravesical growth, open or laparoscopic resection is preferable due to the risk of intraoperative bladder perforation.</p></sec><sec><title>Conclusion</title><p>Conclusion. The presented clinical case demonstrates that transurethral laser enucleoresection is an effective and safe method for bladder leiomyoma treatment, ensuring complete removal with minimal trauma.
