Основным методом оперативного лечения мужчин со стрессовым недержанием мочи средней и тяжёлой степени является установка искусственного мочевого сфинктера (ИМС). Несмотря на эффективность, имплантация сопряжена с развитием осложнений, в ряде случаев требующих ревизий и удаления всего устройства или его компонентов.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Оценить результаты установки ИМС в отношении безопасности и необходимости повторных вмешательств при длительном наблюдении.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. С 2004 по 2023 годы ИМС имплантирован 62 пациентам со стрессовым недержанием мочи тяжёлой степени. Осложнения учитывались согласно модифицированной классификации хирургических осложнений Clavien-Dindo. Производился учёт ревизий, сопровождающихся заменой компонентов или всего ИМС при отсутствии признаков инфекции, и ревизий, сопровождающихся удалением компонентов и всего ИМС в случае инфекции.</p></sec><sec><title>Результаты</title><p>Результаты. Средний возраст пациента на момент имплантации составил 67,4 ± 8 лет. При медиане времени наблюдения 73,5 [14,8 – 118,3] месяца у 22 (35,5%) пациентов отмечены осложнения, выполнены 33 ревизии. Медиана времени до осложнения, требующего замены ИМС или его компонентов, составила 60 [50 – 94] месяцев. Медиана времени до осложнения, требующего удаления ИМС или его компонентов, составила 31 [8 – 83] месяц. Выявлено статистически значимое влияние сахарного диабета на замену компонентов ИМС (ОШ 4,9, p &lt; 0,05).</p></sec><sec><title>Заключение</title><p>Заключение. Установка ИМС связана с развитием осложнений, требующих ревизии, доля их составляет около трети всех вмешательств на ИМС. Ревизии позволяют восстановить работу ИМС и улучшить качество жизни пациентов. С учётом несовершенства применения общехирургической классификации осложнений у мужчин, перенёсших оперативное лечение недержания мочи, требуется пересмотр учёта и классификации осложнений этого вида вмешательств.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Implantation of an artificial urinary sphincter (AUS) is the main treatment method of surgical treatment in men with moderate to severe stress urinary incontinence. Despite its efficacy, implantation is sometimes accompanied by the development of complications, which in some cases necessitate revisions and removal of the entire device or its components.</p></sec><sec><title>Objective</title><p>Objective. To evaluate the outcomes of AUS implantation regarding safety and the need for re-interventions at long-term follow-up.</p></sec><sec><title>Materials &amp; methods</title><p>Materials &amp; methods. From 2004 to 2023, AUS was implanted in 62 patients with severe stress urinary incontinence. Complications are described according to the Clavien-Dindo classification. Revisions followed replacement of components or the entire AUS in the absence of signs of infection and revisions followed components or entire AUS explanation in case of infection were registered.</p></sec><sec><title>Results</title><p>Results. The mean patient age at the time of implantation was 67.4±8years. With amedian follow-up time of 73.5months (IQR 14.8 – 118.3 months), complications were noted in 22 (35.5%) patients, and 33 revisions were performed. The median time to complication requiring replacement of the AUS or its components was 60 months (IQR 50 – 94 months). The median time to complication requiring removal of the AUS or its components was 31 months (IQR 8 – 83 months). A statistically significant effect of type 2diabetes mellitus on AUS component replacement was revealed (OR 4.9, p&lt;0.05).</p></sec><sec><title>Conclusions</title><p>Conclusions. AUS implantation is associated with the development of complications that necessitate revision. These complications account for roughly one-third of all AUS interventions. Revision operations contribute to restoring AUS functionality and enhancing patients' quality of life.
